目的:探讨药剂科在药品不良反应监测管理中存在的问题,并提出针对性的管理建议。方法:对某三甲医院药剂科2015年1~6月间上报的药品不良反应(adverse drug reaction,ADR)信息进行回顾性分析,比较ADR上报信息和实际发生ADR病例情况,分析药剂科在ADR监测管理中存在的主要问题,并给出应对策略。结果:某院药剂科2015年3月~2016年3月间共上报ADR监测信息30份,涉及药品11种;而经查看药剂科发生ADR病例记录情况发现,共有ADR监测信息43份,涉及药品15种,ADR监测信息中,有9份未追踪患者ADR终了情况,有3份由于患者自述而信息不全,2份诊断记录不全,所有未报告病例均已痊愈出院。结论:针对药剂科在ADR的监测管理中暴露的诸多问题,加强规范化管理,提升工作人员对ADR监测工作重要性的认识是应对的主要对策。
OBJECTIVE: To explore the problems existing in the monitoring and management of drug adverse reactions in pharmacy department and to put forward some suggestions on management. Methods: A retrospective analysis was conducted on the adverse drug reaction (ADR) information reported in Pharmacology department of a top three hospitals from January to June, 2015. The information of ADR and the actual incidence of ADR were compared. The data of ADR monitoring Management of the main problems, and gives coping strategies. Results: A total of 30 ADR monitoring information were reported from March 2015 to March 2016 in pharmacy department of a hospital, involving 11 kinds of drugs. According to records of ADR cases found in pharmacy department, there were 43 ADR monitoring information involving drugs 15 kinds of ADR monitoring information, 9 cases did not track patients with ADR end of the situation, 3 due to patient information and incomplete information, 2 diagnostic records are incomplete, all unreported cases have been cured and discharged. Conclusion: In response to many problems exposed by pharmacy department in the monitoring and management of ADR, strengthening the standardized management and raising staff awareness of the importance of ADR monitoring are the main countermeasures.
