目的探讨自然周期法和人工激素替代法对胚胎解冻移植结局的影响。方法收集2014年1月-2015年12月在唐山市妇幼保健院生殖遗传科行胚胎解冻移植的359周期临床资料进行历史性队列研究,按照不同内膜准备方案,将其分为自然周期组(169周期)和人工周期组(190周期),比较不同内膜准备方案妊娠结局差异。结果两组患者年龄、体重指数(BMI)、不孕年限、移植胚胎数及移植优胚数比较,差异均无统计学意义(P>0.05),自然周期组平均内膜厚度、种植率及妊娠率分别为(11.52±2.14)mm、18.1%、33.1%,人工周期组平均内膜厚度、种植率及妊娠率分别为(9.98±1.50)mm、26.3%、49.5%,差异均有统计学意义(P<0.05)。但同一方案不同内膜厚度未对解冻结局造成影响(P>0.05)。结论人工激素替代法行胚胎解冻移植的种植率及妊娠率均高于自然周期法。
Objective To investigate the effect of natural cycle method and artificial hormone replacement method on embryo thawing and transplantation outcome. Methods A total of 359 cycles of embryo thawing and embryo transfer in Tangshan Maternal and Child Health Hospital from January 2014 to December 2015 were collected for historical cohort study. According to the different endomembrane preparations, they were divided into natural cycle group 169 cycles) and artificial cycle group (190 cycles) to compare the differences in pregnancy outcome between different endometrial preparation regimens. Results There were no significant differences in age, body mass index (BMI), duration of infertility, number of embryos transferred and number of embryos transplanted in both groups (P> 0.05). The mean intimal thickness, implantation rate and pregnancy rate (11.52 ± 2.14) mm, 18.1% and 33.1% respectively. The mean intimal thickness, implantation rate and pregnancy rate in the artificial cycle group were (9.98 ± 1.50) mm, 26.3% and 49.5%, respectively, with statistical significance (P <0.05). However, different intima thickness did not affect the thawing outcome in the same protocol (P> 0.05). Conclusion The implantation rate and pregnancy rate of embryo thawed by artificial hormone replacement are higher than those of natural cycle.
