目的:探讨肝内胆管细胞癌(ICC)患者术前外周血中性粒细胞/淋巴细胞(NLR)值与患者远期生存率、生存时间的关系。方法:回顾分析收治的89例ICC患者的诊治经过及临床资料,根据术前患者外周血NLR水平分为50例高NLR(NLR值≥2.5)组和39例低NLR组(NLR值<2.5);对比两组术后生存率、中位时间的差异,并分析影响ICC患者术后预后的影响因素。结果:低NLR患者术后1、3、5年的生存率分别为64.10%、48.72%、38.46%均高于高NLR组患者的42.00%、24.00%、18.00%(P<0.05);低NLR患者的生存时间34个月、无瘤生存时间16个月均高于高NLR组患者的19、9个月(P<0.05);发生淋巴结转移、病灶数多发、病灶直径≥5 cm、血管侵犯、远处转移、姑息手术、NLR值高水平与ICC患者的远期生存时间有关(P<0.05)。结论:ICC患者术前外周血NLR高水平对患者的远期预后将产生不利影响,同时ICC患者的远期预后还受到淋巴结转移、病灶数多发、病灶直径≥5 cm、血管侵犯、远处转移、手术类型的影响。
Objective: To investigate the relationship between preoperative peripheral blood neutrophil / lymphocyte (NLR) values and long-term survival and survival in patients with intrahepatic cholangiocarcinoma (ICC). Methods: The clinical data and clinical data of 89 patients with ICC admitted to our hospital were retrospectively analyzed. According to the level of NLR in preoperative patients, there were 50 patients with high NLR (NLR≥2.5) and 39 patients with low NLR (NLR <2.5) The difference of postoperative survival rate and median time between the two groups were compared, and the influencing factors of postoperative prognosis of ICC patients were analyzed. Results: The survival rates of patients with low NLR at 1, 3 and 5 years after operation were 64.10%, 48.72% and 38.46%, respectively, which were significantly higher than those in patients with high NLR (42.00%, 24.00% and 18.00%, P <0.05) The patient’s survival time was 34 months, and the tumor-free survival time was 16 months higher than that of patients in high NLR group for 19 and 9 months (P <0.05). The incidence of lymph node metastasis, multiple lesion count, lesion diameter> 5 cm, vascular invasion , Distant metastasis, palliative surgery, and high NLR levels were associated with long-term survival time in patients with ICC (P <0.05). Conclusion: The high preoperative NLR of peripheral blood in patients with ICC will have a negative effect on the long-term prognosis of patients. The long-term prognosis of patients with ICC is also affected by lymph node metastasis, multiple lesions, diameter ≥ 5 cm, vascular invasion and distant metastasis , The type of surgery.
