心脏海绵状血管瘤影像学特征与诊断2例报告并文献复习

目的:探讨超声心动图联合其他影像技术诊断心脏海绵状血管瘤的应用价值。方法:回顾性分析我院诊治的心脏海绵状血管瘤患者2例,并结合文献报道的41例,总结该病的流行病学特征、临床表现、超声心动图与其他影像技术特点等。结果:2例患者均经超声心动图发现心脏肿块,1例肿瘤广泛分布于左、右室心肌、房间隔及房壁内,超声造影、MR及CT均提示肿瘤血供丰富,冠状动脉造影提示冠状动脉多支受累,因此未行手术。另1例肿瘤位于三尖瓣后瓣,手术完全切除后经病理证实为海绵状血管瘤。结论:心脏海绵状血管瘤是一种罕见的疾病,超声心动图是首选的影像学评价方法,联合心脏MRI、CT及冠状动脉造影,对诊断该病有很好的帮助。 Objective: To explore the value of echocardiography combined with other imaging techniques in the diagnosis of cardiac cavernous hemangiomas. Methods: Two patients with cardiac cavernous hemangioma diagnosed and treated in our hospital were retrospectively analyzed. Combined with 41 cases reported in the literature, the epidemiological characteristics, clinical manifestations, echocardiography and other imaging techniques of the disease were summarized. Results: Two patients had heart tumor by echocardiography. One tumor was widely distributed in left and right ventricular myocardium, atrial septal and atrial septum. Contrast ultrasound and MR and CT all showed that the tumor was rich in blood supply and coronary angiography Multiple coronary artery involvement, so no surgery. Another case of tumor located in the tricuspid valve flap, after surgical resection completely confirmed by pathology cavernous hemangioma. Conclusion: Cardiac Cavernous Hemangioma is a rare disease. Echocardiography is the first imaging evaluation method. Combined with cardiac MRI, CT and coronary angiography, it is helpful to diagnose the disease.
