目的探讨应用心肌速度阶差评价无并发症糖尿病患者左心室功能情况。方法收集确诊为无并发症糖尿病患者100例作为试验组,同期收集体检健康的志愿者100例作为对照组。2组均行常规超声心动图检测左心室结构,运用心肌速度分布图测定心肌运动速度,计算左心室射血分数(LVEF)、左心室收缩期和舒张期心肌速度阶差,比较2组间差异。结果试验组患者左房前后径(LA)、左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)、舒张期室间隔厚度(IVSTD)均大于对照组,而左室射血分数(LVEF)、左心室后壁收缩期心肌速度阶差(MVGs)和左心室后壁舒张期心肌速度阶差(MVGd)小于对照组,差异均有统计学意义(P<0.05);但舒张期左室后壁厚度(LVPWTD)、室壁相对厚度(RWT)与对照组比较,差异无统计学意义(P>0.05)。结论心肌速度阶差评价发现无并发症糖尿病患者左心室功能已存在心室结构改变、心肌肥厚、心功能减退等心脏病征象。
Objective To investigate the assessment of left ventricular function in patients with diabetes mellitus without concomitant complications by using myocardial velocity gradient. Methods 100 cases diagnosed as diabetic patients without complications were collected as experimental group and 100 healthy volunteers were collected as control group. The left ventricular structure was measured by routine echocardiography in both groups. The myocardial velocity was measured by the myocardial velocity distribution chart, and the left ventricular ejection fraction (LVEF), left ventricular systolic and diastolic myocardial velocity were calculated. The differences between the two groups were compared . Results The left anterior chamber diameter (LA), left ventricular end diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) and diastolic interventricular septum thickness (IVSTD) in the experimental group were significantly higher than those in the control group LVEF, MVGs, and MVGd in the left ventricular wall were less than those in the control group (P <0.05). However, left ventricular diastolic left There was no significant difference in LVPWTD and RWT between the two groups (P> 0.05). Conclusions The evaluation of myocardial velocity step difference found that left ventricular function in patients with diabetes mellitus had cardiac structural changes, cardiac hypertrophy and cardiac dysfunction.
