钼靶X线、常规超声及其联合声触诊组织量化技术对乳腺实性肿块的诊断价值

目的:探究钼靶X线、常规超声及其联合声触诊组织量化(VTQ)技术对乳腺实性肿块的诊断价值。方法:回顾性分析120例患者(132个乳腺实性病灶)的临床资料。患者均经手术或穿刺组织病理确定肿块病理性质,术前均接受钼靶X线、常规超声及VTQ检查。采用美国放射学会发布的乳腺影像报告与数据系统(BI-RADS)对病灶检查结果进行分类,用VTQ值对病灶常规超声BI-RADS分类进行调整。评价钼靶X线、常规超声及其联合VTQ技术诊断乳腺实性肿块的诊断效能。结果:132个病灶中,良性86个,恶性46个,良性肿块以纤维腺瘤为主,恶性肿块以浸润性导管癌为主。VTQ诊断的最佳界值为4.35 m/s,钼靶X线、常规超声及联合检查的受试者工作特征曲线下面积(AUC)分别为0.771、0.799、0.904,联合检查的AUC大于钼靶X线及常规超声(n P0.05)。联合检查的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为91.30%、89.53%、90.15%、82.35%、95.06%。n 结论:在乳腺实性肿瘤诊断中,常规超声、钼靶X线诊断效能相当,而常规超声联合VTQ技术可提高诊断效能,具有较高应用价值。“,”Objective:To investigate the diagnostic value of molybdenum target X-ray, conventional ultrasound, and combination of ultrasound with virtual touch tissue quantification (VTQ) for solid breast masses.Methods:The clinical data from 120 cases of female patients with 132 breast solid masses were retrospectively analyzed. The pathological properties of masses were determined by surgery. Before surgery, molybdenum target X-ray, conventional ultrasound, and VTQ were performed on all patients. And breast imaging reporting and data system (BI-RADS) was used to classify the lesion examination result. And the BI-RADS classification of conventiinal ultrasound was adjusted according to VTQ value combined conventional ultrasound. The diagnosis efficiency of molybdenum target X-ray, conventional ultrasound and combination of them with VTQ technology in the diagnosis of breast masses was evaluated.Results:Among the 132 lesions, 86 cases were benign, and 46 cases were malignant. The benign masses mainly were fibroadenomas, and the malignant masses mainly were invasive ductal carcinoma. The optimal threshold of VTQ diagnosis was 4.35 m/s, and the area under receiver operating characteristic curve (AUC) of molybdenum target X-ray, conventional ultrasound, and combined examination were 0.771, 0.799 and 0.904, respectively. The AUC of combined examination was greater than that of molybdenum target X-ray and conventional ultrasound (n P0.05). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of combined examination were 91.30%, 89.53%, 90.15%, 82.35% and 95.06%, respectively.n Conclusions:Conventional ultrasound and molybdenum target X-ray are equally effective in the diagnosis of breast solid tumor, while conventional ultrasound combined with VTQ can improve the diagnostic efficiency and has high application value.
