目的 研究 p5 3、bcl 2及PCNA在大肠肿瘤发生发展中的作用及相互关系。方法 应用免疫组化S P法分别检测大肠正常粘膜、增生性息肉、腺瘤及腺癌中p5 3、bcl 2及PCNA蛋白表达。结果 正常粘膜及增生性息肉中 p5 3表达阴性 ,腺瘤癌变及腺癌组 p5 3蛋白表达率明显高于腺瘤组(P <0 0 5 )。腺瘤和腺癌中bcl 2表达差异显著 (P <0 0 5 )。高分化腺癌bcl 2表达率高于差分化腺癌 (P <0 0 1)。bcl 2与p5 3表达在腺瘤和腺癌中呈负相关。PCNA强阳性表达在大肠腺瘤 -癌序列中逐渐增高 ,它与腺瘤 p5 3表达、不典型增生程度及腺癌组织分化有关 (P <0 0 5 )。结论 p5 3基因突变主要涉及到腺瘤向腺癌的转变过程 ,大肠癌的发生与腺瘤中凋亡相对抑制和增殖活性增高有关。综合分析 p5 3、bcl 2和PCNA表达有助于大肠癌的早期诊断和预后判断
Objective To investigate the roles and relationships of p5 3, bcl 2 and PCNA in the development of colorectal tumors. Methods The expressions of p5 3, bcl 2 and PCNA in normal mucosa, hyperplastic polyps, adenoma and adenocarcinoma were detected by immunohistochemical SP method. Results The expression of p5 3 protein in normal mucosa and proliferative polyp was negative, and the expression of p5 3 in adenocarcinoma and adenocarcinoma was significantly higher than that in adenoma (P <0.05). The expression of bcl 2 in adenoma and adenocarcinoma was significantly different (P <0.05). The expression of bcl-2 in well-differentiated adenocarcinoma was higher than that in differentiated adenocarcinoma (P <0.01). The expression of bcl 2 and p5 3 was negatively correlated with adenoma and adenocarcinoma. The strong positive expression of PCNA in colorectal adenoma - cancer sequence was gradually increased, which was related to the expression of p53, the degree of dysplasia and the differentiation of adenocarcinoma (P <0.05). Conclusion The mutation of p5 3 mainly involves the process of adenoma transition to adenocarcinoma. The occurrence of colorectal carcinoma is related to the relative inhibition of apoptosis and the increase of proliferative activity in adenoma. Comprehensive analysis of p5 3, bcl 2 and PCNA expression contribute to the early diagnosis and prognosis of colorectal cancer
