目的:观察归龙汤对糖尿病性足病患者血液流变学及外周内皮细胞的影响.方法:纳入160例糖尿病性足病患者作为研究对象,采用随机数表法分为两组,各80例.对照组行常规依帕司他干预治疗,观察组在此基础上加用归龙汤.比较两组治疗效果、血液流变学指标(全血高切粘度、全血低切粘度、血浆粘度、全血还原粘度、红细胞压积)及外周血内皮细胞功能指标[内皮细胞微粒(EMPs)、可溶性细胞间粘附因子-1(sICAM-1)和血浆von Will-brand因子(wWF)].探讨归龙汤临床疗效及作用机制.结果:观察组治疗总有效率为95%,对照组为80%,组间比较差异有显著性差异(P<0.05).观察组治疗后全血高切粘度和全血低切粘度分别为(5.19±0.38).μg/分钟和(7.43±0.69)μg/分钟,显著低于治疗前和对照组治疗后(P<0.05).观察组治疗后EMPs、sICAM-1及wWF水平显著低于治疗前和对照组治疗后(P<0.05).结论:归龙汤能显著改善患者肢体疼痛、麻木症状,这可能与归龙汤改善患者血液流变学和外周血内皮细胞功能有关.“,”Objective:To study the effect of Guilong Decoction on hemorheology and peripheral endothelial cells in patients with diabetic foot.Methods:A total of 160patients with diabetic foot were randomly divided into two groups,80cases in each.The controlled group was given routine epalrestat intervention,while the observation group was additionally treated with Guilong Decoction.The clinical effect,hemorheological indexes (whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,whole blood reduction viscosity,hematocrit) and peripheral blood endothelial cell function indexes [endothelial microparticles (EMPs) and soluble intercellular adhesion molecule-1 (sICAM-1) and plasma yon Willbrand factor (wWF)] were compared between the two groups.The therapeutic effect and mechanism of Guilong Decoction was explored.Results:The total effective rate in the observation group was 95% while in the controlled group was 80% (P < 0.05).After treatment,the whole blood high shear viscosity and whole blood low shear viscosity of the observation group [(5.19±0.38) μg/min and (7.43± 0.69) μg/min] were significantly lower than those before treatment and those in the controlled group after treatment (P < 0.05).After treatment,the levels of EMPs,sICAM-1and wWF in the observation group were significantly lower than those before treatment and those in the controlled group after treatment (P < 0.05).Conclusion:Guilong Decoction can significantly relieve limb pain and numbness,which may be related to the improvement of hemorheology and peripheral blood endothelial cell function.
