目的:探究不同睡眠呼吸暂停低通气指数(AHI)患者各类精神行为的严重程度差异,为睡眠呼吸暂停综合症的诊断与治疗提供理论依据。方法:对2011年11月-2015年11月于我院干预治疗的100例睡眠呼吸暂停综合症患者进行相关精神症状的调查,将睡眠呼吸暂停低通气指数表现为重度的50例患者设为观察组。将呼吸暂停低通气指数表现为轻度以及中度的50例患者设为对照组。比较两组患者记忆力下降、困倦、情绪低落、脾气暴躁的发生率,同时对焦虑(SAS)以及抑郁(SDS)评分进行比较,通过症状自评量表SCL-90评价两组患者自觉精神症状的严重程度进行比较。结果:观察组患者记忆力下降、情绪低落、困倦、脾气暴躁发生率分别为70.00%、68.00%、74.00%、66.00%,对照组分别为34.00%、36.00%、28.00%、38.00%,观察组均显著高于对照组,有统计学意义(χ~2=12.98,10.26,21.17,7.85;P<0.01);观察组患者SAS、SDS评分均显著高于对照组,有统计学意义(t=29.75,7.82;P<0.001);观察组SCL-90中恐怖、偏执、敌意评分与对照组无显著差异,精神病性、抑郁、焦虑、人际关系敏感、强迫症、躯体化、其他评分与对照组差异显著,有统计学意义(t=32.80,12.87,16.19,14.09,14.29,15.52,43.41;P<0.001)。结论:睡眠呼吸暂停综合症患者精神状况较差、焦虑、抑郁症状较为明显,同时睡眠呼吸暂停综合症高的患者各精神行为异常症状较程度低者偏高,因此在睡眠呼吸暂停综合症的日常诊治中应加强对精神行为异常的关注。
Objective: To explore the differences in the severity of various mental behaviors in patients with different sleep apnea-hypopnea index (AHI), and to provide a theoretical basis for the diagnosis and treatment of sleep apnea syndrome. Conclusion: The patients with sleep apnea syndrome have poor mental status, obvious anxiety and depressive symptoms, and all the patients with high sleep apnea syndrome have higher mental abnormalities than those with low degree.
