视网膜母细胞瘤化疗前后血清乳酸脱氢酶水平的变化及意义

目的探讨视网膜母细胞瘤化疗前后血清乳酸脱氢酶(LDH)的变化及其对化疗疗效的评价。方法回顾性分析31例接受CEV方案(卡铂+依托泊苷+长春新碱)化疗的视网膜母细胞瘤患者,测定化疗前后血清LDH水平,行眼底检查对肿瘤分期并记录化疗前后的肿瘤变化。分析LDH、性别、年龄、眼别及肿瘤分期与化疗疗效的关系。结果化疗前血清LDH为255.7U/L,化疗后为229.1U/L,差异有统计学意义(t=2.748,P=0.01)。化疗后血清LDH水平降低者的疗效高于LDH水平升高者,差异有统计学意义(χ2=6.339,P=0.042);分期为D期以下者的疗效高于分期为D期及以上者,差异有统计学意义(χ2=4.895,P=0.041);而年龄、眼别及性别对化疗疗效无影响。结论视网膜母细胞癌化疗前后血清LDH变化及肿瘤分期是影响化疗疗效的重要因素,可以作为疗效判断的参考评价指标。 Objective To investigate the changes of serum lactate dehydrogenase (LDH) in patients with retinoblastoma before and after chemotherapy and its effect on chemotherapy. Methods 31 patients with retinoblastoma who underwent chemotherapy with carboplatin and etoposide plus vincristine were retrospectively analyzed. Serum LDH levels were measured before and after chemotherapy. The tumor staging was recorded by fundus examination and the changes of tumor before and after chemotherapy were recorded. Analysis of LDH, sex, age, eye and tumor staging and the relationship between the efficacy of chemotherapy. Results Before chemotherapy, the serum LDH was 255.7U / L and 229.1U / L after chemotherapy, the difference was statistically significant (t = 2.748, P = 0.01). Chemotherapy after the reduction of serum LDH levels were higher than those with elevated LDH levels, the difference was statistically significant (χ2 = 6.339, P = 0.042); staging for the D period was higher than those with stage D and above, The difference was statistically significant (χ2 = 4.895, P = 0.041); while age, eye and sex had no effect on the efficacy of chemotherapy. Conclusion The changes of serum LDH and tumor staging before and after chemotherapy for retinoblastoma are important factors affecting the curative effect of chemotherapy, which can be used as a reference evaluation index for curative effect judgment.
