显微外科技术在模拟海战四肢创伤修复与重建的动物实验研究

目的通过建立犬肢体伤断后经不同时间海水浸泡的动物模型,在野战医院或医院舰船实施严格清创手术,研究应用显微外科技术进行修复与重建的可能性与优点。方法杂种家犬24只,分野战医院及医院舰船两组,手术致伤断,检测海水温度,各经1、2、3 h海水浸泡后送达野战医院或即在医院船手术,应用显微外科技术进行修复与重建,术后对肢体着地,神经电生理、肌腱超声、X线摄片及组织学进行检测。研制圆形铁板及U形磁钢在舰船上固定手术显微镜以防滑移。结果犬肢体断伤后经海水浸泡,随着海水温度降低术后犬死亡率也明显增高;上述各项指标检测结果与人体大致相似,实验分组及海水浸泡时间对检测结果有一定影响。结论海战时在舰船上实施显微外科的手术是可行的,其术后效果明显优于常规外科治疗;圆形铁板及U形磁钢固定手术显微镜是一项重要技术革新,具有制作简单、携带拆卸方便、固定牢靠的优点;海战时伤员落水,在医院船或救护艇就地实施救治具有无可比拟的优点。 OBJECTIVE: To establish the animal model of canine limbs injury after immersion in seawater at different times, to carry out strict debridement surgery in field hospital or hospital warship, and to study the possibility and advantages of using microsurgical techniques to repair and reconstruct. Methods Twenty-four hybrid dogs were divided into two subgroups: field operation hospital and hospital warship. The operation was broken and the seawater temperature was measured. Each seawater was immersed in seawater for 1, 2 and 3 h, and then delivered to a field hospital or a hospital ship operation. The micro-surgical technique was used to repair and reconstruct the limbs. The limbs, the electrophysiology, tendon ultrasound, X-ray and histology were detected after operation. Results The dog limbs were immersed in seawater after breaking, and the postoperative dog mortality rate was also significantly increased with the decrease of seawater temperature. The results of the above indexes were similar to those in human body. The experimental group and seawater immersion time had certain influence on the test results. Conclusion It is feasible to carry out microsurgical operations on ships during naval warfare, and its postoperative effect is obviously better than that of conventional surgical treatment.
