目的探讨无保护会阴助产技术在促进自然分娩中的应用效果。方法选取2015年1—6月上饶市人民医院妇产科收治的自然分娩产妇130例,随机分为观察组与对照组,各65例。观察组产妇予以无保护会阴助产技术,对照组产妇予以常规助产技术。观察两组产妇产后会阴情况(会阴侧切率、会阴完整率、会阴Ⅰ度裂伤率、会阴Ⅱ度裂伤率)、并发症(会阴水肿、排尿困难、尿潴留)发生情况、新生儿窒息率、第二产程时间、产时出血量、产后出血量、疼痛评分及住院时间。结果观察组产妇会阴侧切率、会阴Ⅱ度裂伤率低于对照组,会阴完整率、会阴Ⅰ度裂伤率高于对照组,差异有统计学意义(P<0.05);观察组产妇会阴水肿、排尿困难、尿潴留发生率低于对照组,差异有统计学意义(P<0.05);两组新生儿窒息率比较,差异无统计学意义(P>0.05);两组产妇第二产程时间比较,差异无统计学意义(P>0.05);观察组产妇产时出血量、产后出血量少于对照组,疼痛评分低于对照组,住院时间短于对照组,差异有统计学意义(P<0.05)。结论无保护会阴助产技术在促进自然分娩中的应用效果显著,可降低产妇会阴侧切率及并发症发生率,促进产妇产后恢复。 Objective To explore the effect of unprotected perineal midwifery technology in promoting spontaneous labor. Methods 130 cases of natural childbirth received by Department of Obstetrics and Gynecology, Shangrao People’s Hospital from January to June in 2015 were randomly divided into observation group and control group, with 65 cases in each group. Observation group of mothers to unprotected perineal midwifery technology, the control group of mothers to be routine midwifery technology. The incidence of perineal episodes (episiotomy lateralization rate, perineal completeness rate, perineal first degree laceration rate, perineal second degree laceration rate) and complications (perineal edema, dysuria, urinary retention) were observed in two groups. The neonatal asphyxia rate , The second stage of labor time, the amount of bleeding during delivery, postpartum hemorrhage, pain score and length of stay. Results In the observation group, the rate of perineal episiotomy and the degree of laceration of perineal second degree were lower than that of the control group. The perineal complete rate and perineal first-degree laceration rate were higher in the observation group than in the control group (P <0.05). The perineal edema , Dysuria and urinary retention were lower than those in the control group (P <0.05). There was no significant difference in neonatal asphyxia between the two groups (P> 0.05) (P> 0.05). The amount of bleeding during labor, the amount of postpartum hemorrhage in the observation group were lower than those in the control group, the pain score was lower than that in the control group, and the length of stay in hospital was shorter than that in the control group P <0.05). Conclusions The unprotected perineal midwifery technology has significant effect in promoting spontaneous labor and can reduce the incidence of perineal percussion side and rate of complications and promote the recovery of postpartum women.
