目的:分析农村中老年人群高甘油三酯腰围(HTGW)、2型糖尿病(T2DM)的流行现状及两者间的关联。方法:采用整群抽样的方法于2013年和2014年的7~8月对河南某农村地区45~75岁的中老年人群进行横断面调查,收集其人口统计学特征、行为危险因素、人体测量指标和空腹血标本,并进行血糖和脂质谱的检测。结果:共纳入10 218名研究对象,其中男性3 836人(37.54%),中位年龄56.62岁。HTGW共2 845人(27.84%),男性646人(16.84%),女性2 199人(34.46%)。T2DM共1 406人(13.76%),男性469人(12.23%),女性937人(14.68%)。在调整了年龄、血压、吸烟、饮酒、家族史、体力活动、体重指数(BMI)、高密度脂蛋白后,logistic回归分析结果显示与腰围和甘油三酯水平均正常的人群相比,HTGW可增加T2DM的发病风险,其OR(95%CI)为3.810(3.050~4.760)。以性别和BMI为分层变量分析发现,HTGW与T2DM的关联女性高于男性、18.5≤BMI<24.0kg/m~2人群高于24.0≤BMI<28.0 kg/m~2人群。结论:该农村地区中老年人群HTGW和T2DM的患病率较高,HTGW是T2DM稳定的危险因素。
Objective: To analyze the prevalence of hypertriglyceridemia (HTGW) and type 2 diabetes mellitus (T2DM) in rural middle-aged and elderly people and their relationship. Methods: By means of cluster sampling, from July to August in 2013 and 2014, a cross-sectional survey was conducted on middle-aged and aged people in 45-75 years old in a rural area in Henan Province. The demographic characteristics, behavioral risk factors, Indicators and fasting blood samples, and blood glucose and lipid profile detection. Results: A total of 10 218 study subjects were enrolled, including 3 836 males (37.54%) with a median age of 56.62 years. There were 2845 (27.84%) HTGWs, 646 (16.84%) males and 2 199 (34.46%) females. There were 1 406 (13.76%) T2DM patients, 469 (12.23%) men and 937 (14.68%) females. After adjusted for age, blood pressure, smoking, alcohol consumption, family history, physical activity, body mass index (BMI), and high density lipoprotein, logistic regression analysis showed that HTGW was significantly higher than those with normal waist circumference and triglyceride levels Increasing the risk of T2DM, the OR (95% CI) was 3.810 (3.050 ~ 4.760). Hierarchical analysis of gender and BMI found that the association of HTGW with T2DM was higher in females than in females, with 18.5 ≤ BMI <24.0 kg / m ~ 2 higher than 24.0 ≤ BMI <28.0 kg / m ~ 2. Conclusion: The prevalence of HTGW and T2DM in middle-aged and elderly people in this rural area is high, and HTGW is a risk factor for stable T2DM.
