目的:探讨黄芩汤辅助柳氮磺吡啶对溃疡性结肠炎患者血清TNF-α及白介素族水平的影响。方法:选取研究对象为74例溃疡性结肠炎患者,随机分成对照组和治疗组,每组37例。对照组给予柳氮磺吡啶治疗,治疗组在上述基础上再予黄芩汤治疗,两组均治疗1个月,测定治疗前后血清白介素族及TNF-α,免疫球蛋白水平和结肠黏膜菌群,观察疗效及安全性。结果:与治疗前比较,两组血清IL~(-1)β、IL-6及TNF-α水平明显降低(P<0.05),IL~(-1)0水平明显升高(P<0.05),血清Ig A及Ig G水平明显降低(P<0.05),结肠黏膜双歧杆菌及乳酸菌数量明显升高(P<0.05),大肠杆菌数量明显降低(P<0.05);与对照组比较,治疗组血清IL~(-1)β、IL-6及TNF-α水平较低(P<0.05),IL~(-1)0水平较高(P<0.05),血清Ig A及Ig G水平较低(P<0.05),结肠黏膜双歧杆菌及乳酸菌数量较高(P<0.05),大肠杆菌数量较低(P<0.05),治疗有效率较高(P<0.01),不良反应发生率较低(P<0.05)。结论:黄芩汤辅助柳氮磺吡啶能提高对溃疡性结肠炎的疗效,其可能机制为调整肠道菌群,调节免疫以及不同血清白介素的水平。
Objective: To investigate the effects of Huangqin Decoction on sulfasalazine on serum levels of TNF-α and IL-1 in patients with ulcerative colitis. Methods: 74 patients with ulcerative colitis were selected and randomly divided into control group and treatment group, 37 cases in each group. The control group was treated with sulfasalazine. The treatment group was treated with Huangqin Decoction on the above basis. Both groups were treated for 1 month. Serum levels of IL-1, TNF-α, immunoglobulin and colonic mucosal flora were measured before and after treatment. To observe the efficacy and safety. Results: The levels of IL - 1, IL - 6 and TNF - α in the two groups were significantly lower than those before treatment (P <0.05), while the levels of IL - (P <0.05). The numbers of Bifidobacterium and Lactobacillus in colonic mucosa were significantly increased (P <0.05) and the number of Escherichia coli was significantly decreased (P <0.05). Compared with the control group, the levels of IgA and Ig G in serum were significantly decreased Serum levels of IL-1β, IL-6 and TNF-α were lower (P <0.05), serum IL-1 (0) was higher (P <0.05) (P <0.05). The number of Bifidobacterium and Lactobacillus in colonic mucosa was higher (P <0.05), the number of Escherichia coli was lower (P <0.05), the treatment was more efficient (P <0.01), and the incidence of adverse reactions was Low (P <0.05). Conclusion: Huangqin decoction can ameliorate the effect of sulfasalazine on ulcerative colitis. Its possible mechanism is to regulate intestinal flora, regulate immunity and serum interleukin levels.
