目的探讨浙东南地区上消化道疾病与幽门螺杆菌感染(Helicobacter pylori,H.pylori)的关系。方法对2010年1月至2013年12月期间该地区接受胃镜检查同时行幽门螺杆菌检查14 083例患者进行回顾性分析,分析该地区不同上消化道疾病H.pylori感染情况。结果总共发现6 133例H.pylori感染,感染率为43.5%,食管炎H.pylori感染率为37.9%,Barrett食管H.pylori感染率为36.4%,非萎缩性胃炎H.pylori感染率为41.7%,萎缩性胃炎H.pylori感染率为43.0%,胃癌H.pylori感染率为54.7%,胃溃疡H.pylori感染率为64.4%,十二指肠溃疡H.pylori感染率为70.6%。胃溃疡、十二指肠溃疡和胃癌H.pylori感染率均高于总体感染率(P<0.05),食管炎、Barrett食管和非萎缩性胃炎H.pylori感染率均低于总体感染率(P<0.05)。上述上消化道疾病H.pylori感染率性别比较差异均无统计学意义(P>0.05)。结论浙东南地区消化性溃疡(包括胃溃疡和十二指肠溃疡)H.pylori感染率最高,最低的是食管炎和Barrett食管,H.pylori感染率无性别差异。
Objective To investigate the relationship between upper gastrointestinal diseases and Helicobacter pylori (Helicobacter pylori) in southeastern Zhejiang Province. Methods A retrospective analysis of 14 083 patients who underwent gastroscopy and Helicobacter pylori in this area from January 2010 to December 2013 was performed to analyze the prevalence of H.pylori infection in different upper gastrointestinal diseases in this area. Results A total of 6 133 cases of H.pylori infection were found, the infection rate was 43.5%, the infection rate of H.pylori in esophagitis was 37.9%, the infection rate of H.pylori in Barrett’s esophagus was 36.4%, and the infection rate of H.pylori in non-atrophic gastritis was 41.7 The prevalence of H.pylori in atrophic gastritis was 43.0%, the infection rate of H.pylori in gastric cancer was 54.7%, the infection rate of H.pylori in gastric ulcer was 64.4%, and the infection rate of H.pylori in duodenal ulcer was 70.6%. Gastric ulcer, duodenal ulcer and H.pylori infection rate of gastric cancer were higher than the overall infection rate (P <0.05), esophagitis, H.pylori infection of Barrett’s esophagus and non-atrophic gastritis were lower than the overall infection rate (P <0.05). There were no significant differences in the H.pylori infection rates between the above upper gastrointestinal diseases (P> 0.05). Conclusion The prevalence of H.pylori in peptic ulcer (including gastric ulcer and duodenal ulcer) in southeastern Zhejiang Province is the highest, the lowest is esophagitis and Barrett’s esophagus, and there is no gender difference in H.pylori infection rate.
