目的 研究脑梗死患者血糖增高的机制及其对病情的影响 ,为临床治疗脑梗死提供依据。方法 收集 1995~ 1998年 64例脑梗死患者和 60例对照者 ,动态测定血糖、ACTH、皮质醇、胰高血糖素和C 肽的含量 ,进行回归分析。结果 病人组比对照组血糖、皮质醇、C 肽明显增高 (P <0 .0 1) ,ACTH、胰高血糖素明显降低 (P <0 .0 1)。结论 回归分析表明 ,作为血糖调解的主因胰岛素 ,在脑梗死时被皮质醇取而代之。
Objective To study the mechanism of elevated blood glucose in patients with cerebral infarction and its effect on the disease, so as to provide basis for clinical treatment of cerebral infarction. Methods Sixty-four patients with cerebral infarction and 60 controls were collected from 1995 to 1998. The levels of blood glucose, ACTH, cortisol, glucagon and C-peptide were determined dynamically and analyzed by regression analysis. Results The blood glucose, cortisol and C-peptide in patients group were significantly higher than those in control group (P <0.01), while ACTH and glucagon were significantly lower (P <0.01). Conclusions Regression analysis showed that insulin, the main cause of blood glucose mediation, was replaced by cortisol in cerebral infarction.
