目的:探讨当归拈痛汤及其拆方对急性痛风性关节炎大鼠血尿酸的影响并观察大鼠肾脏脏器指数及关节滑膜的病理改变。方法:采用腺嘌呤和乙胺丁醇造成大鼠体内高尿酸血症模型,在大鼠高尿酸前提下,采用尿酸钠(MUS)诱导大鼠急性痛风性关节炎模型。将60只大鼠随机分为正常组、模型组、全方组、茵陈组、羌活组、西药组,每组10只。正常组、模型组均予生理盐水治疗,全方组、茵陈组、羌活组、西药组分别予当归拈痛汤、当归拈痛汤拆方1、当归拈痛汤拆方2、秋水仙碱治疗。测定各组大鼠血尿酸值及肾脏脏器指数,并观察大鼠关节滑膜的病理改变。结果:与模型组比较,西药组、茵陈组、羌活组、全方组血尿酸值、肾脏脏器指数均下降(P<0.01或P<0.05);与西药组比较,全方组、茵陈组、羌活组血尿酸值、肾腺脏器指数升高(P<0.05);茵陈组血尿酸值下降较羌活组明显(P<0.05);茵陈组、羌活组间肾脏脏器指数差异无统计学意义(P>0.05);全方组及西药组可明显减少关节滑膜中炎性细胞浸润,减轻急性痛风性关节炎大鼠关节滑膜的充血及肿胀情况;茵陈组减轻滑膜组织肿胀作用较好,羌活组炎性细胞浸润较少。结论:当归拈痛汤全方改善急性痛风性关节炎大鼠血尿酸值、降低肾脏指数、减轻滑膜病理变化作用优于当归拈痛汤拆方1、当归拈痛汤拆方2。“,” Objective: To investigate the effects of Chinese Angelica Pain-Assuaging Decoction and its disassembled de-coction on serum uric acid, renal organ index and joint synovium in rats. Methods: Hyperuricemia model in Rats was repli-cated by injection of adenine and ethambutol. 60 SPF rats were randomly divided into normal group, model group, Chinese Angelica Pain-Assuaging Decoction group, Siphonostegia group, Notopterygium group and Western Medicine group, 10 rats in each group. Rats were given physiological saline in normal group and model group by intragastric administration, Chinese An-gelica Pain-Assuaging Decoction in Chinese Angelica Pain-Assuaging Decoction group, disassembled decoction 1 in Siphonos-tegia group, disassembled decoction 2 in Notopterygium group, and colchicine in Western Medicine group. The uric acid lev-els, Kidney index and changes of synovium were observed. Results: Compared with the modlel group, the levels of uric acid and kidney index in Western Medicine group, Chinese Angelica Pain-Assuaging Decoction group, Siphonostegia group and No-topterygium group decreased markedly(P<0.01 or P<0.05); Compared with the Western Medicine group, the levels of uric acid and kidney index in Chinese Angelica Pain-Assuaging Decoction group, Siphonostegia group and Notopterygium group in-creased (P<0.05); the levels of uric acid in Siphonostegia group was lower than that of Notopterygium group (P0.05). The Western Medicine group and the Chinese Angelica Pain-Assuaging Decoction group can reduce the infiltration of inflammatory cells in synovial joints congestion, as well as swelling of synovial acute gouty arthritis in rats. the Siphonoste-gia group was good at reducing synovial swelling degree; there was less inflammatory cell infiltrations in Notopterygium group . Conclusion: Chinese Angelica Pain-Assuaging Decoction can decrease the uric acid, kidney index and the changes of syn-ovium, and the effect is better than that of the Chinese Angelica Pain-Assuaging Decoction disassembled decoction 1 and 2.
