目的 探讨原发性开角型青光眼 (primaryopenangleglaucoma ,POAG)的光学相干断层成像术 (opticalcoherencetomography ,OCT)的图像特征 ,POAG患者视网膜神经纤维层 (retinalnervefiberlayer,RNFL)厚度与正常人之间的差异 ,RNFL与视野指数之间的关系。方法 采用OCT对 83例(149只眼 )POAG患者和 83例 (15 0只眼 )正常人进行以视乳头中心为圆心、直径为 3 46mm的环形扫描 ,观察正常人和POAG患者OCT图像特征 ;将正常人与POAG患者各象限和平均RNFL厚度的均数进行比较 ,将各期POAG的RNFL厚度进行比较 ;将POAG 115只眼的平均RNFL厚度与视野指数进行直线回归和相关分析 ;并分析OCT检测POAG患者RNFL厚度的敏感性与特异性。结果 正常人RNFL的OCT图像特征为上、下极厚 ,其次为颞侧 ,鼻侧最薄。曲线表现为双峰形。POAG患者的OCT图像可表现为局限性变薄或缺损、弥漫性变薄、弥漫性变薄并局限性缺损。正常人颞侧、上方、鼻侧、下方和平均RNFL厚度分别为 (90 1± 10 8) μm、(140 4± 10 5 ) μm、(85 2± 14 0 ) μm、(140 4± 9 7)μm、(114 2± 6 0 ) μm ,POAG患者相应的RNFL厚度分别为 (5 6 0± 31 0 ) μm、(81 0± 36 3) μm、(4 7 1± 2 7 5 ) μm、(73 4± 38 4) μm、(6 4 6± 2 8 8) μm ,差异?
Objective To investigate the imaging features of optical coherence tomography (OCT) in patients with primary open angle glaucoma (POAG) and the difference between retinal nerve fiber layer (RNFL) thickness and POAG patients. RNFL And the relationship between visual field index. Methods Eighty - three POAG patients (149 eyes) and 83 normal controls (15 eyes) with OCT were enrolled in the study. The OCT images of normal subjects and POAG patients were observed by circular scan with a center of optic disc and a diameter of 466 mm. The mean RNFL thickness was compared between each quadrant of normal and POAG patients and the average RNFL thickness of each POAG. The mean RNFL thickness and visual field index of 115 eyes were analyzed by linear regression and correlation analysis. The OCT Sensitivity and specificity of detecting RNFL thickness in patients with POAG. Results The OCT images of RNFL in normal subjects were characterized by thick top and bottom, followed by temporal and nasal thinnest. The curve shows a bimodal shape. OCT images of patients with POAG may show localized thinning or defects, diffuse thinning, diffuse thinning, and localized defects. The mean temporal RNFL thickness of the normal subjects were (90 1 ± 10 8) μm, (140 4 ± 10 5) μm, (85 ± 14 0) μm, (140 4 ± 9 7) ) and (114 2 ± 60) μm respectively. The corresponding RNFL thicknesses of POAG patients were (56 ± 31) μm, (81 ± 36 3) μm and (471 ± 2757) μm, (73 4 ± 38 4) μm and (6 4 6 ± 2 8 8) μm, respectively.
