目的 研究肾小管细胞溶酶体损伤在顺铂所致实验性急性肾功能衰竭 (ARF)发病中的作用 ,探讨 8种L -氨基酸合剂的肾功能保护作用及机理。方法 雄性Sprague -Dawley鼠静脉注射 2ml氨基酸合剂或 0 9%氯化钠液后接受 10mg/kg顺铂 ,随后氨基酸或氯化钠以 2ml/h维持 3小时 ,测定尿NAG活性 ,尿钠、钾浓度 ,GFR及做形态学观察。结果 顺铂注射后 30分钟 ,尿NAG活性增加 7 8倍 ,尿钠、钾浓度分别增加 5 6 %及 2 6倍。应用该氨基酸合剂使增高的尿NAG活性减少76 % ,改善肾小管钠、钾排泄异常 ,增加GFR 85 %。结论 溶酶体损伤可能在顺铂所致实验性ARF发生极早期阶段起重要作用 ,该氨基酸合剂有明显的肾功能保护作用 ,其机理可能是通过改善受损的溶酶体功能。
Objective To investigate the role of renal cell lysosome damage in the pathogenesis of experimental acute renal failure (ARF) induced by cisplatin and to explore the renal protective effects and mechanism of 8 kinds of L - amino acid mixture. Methods Male Sprague-Dawley rats received intravenous injection of 2ml of amino acid mixture or 0.9% sodium chloride solution and received 10mg / kg of cisplatin. The amino acids or sodium chloride was then maintained at 2ml / h for 3 hours. Urinary NAG and urinary sodium Concentration, GFR and morphology were observed. Results 30 minutes after cisplatin injection, urinary NAG activity was increased by 78% and urinary sodium and potassium concentrations were increased by 56% and 26% respectively. The application of the amino acid mixture can increase the activity of urinary NAG by 76%, improve renal tubular sodium and potassium excretion, increase GFR 85%. Conclusion Lysosomal damage may play an important role in the very early stage of experimental ARF induced by cisplatin. The amino acid mixture has obvious protective effect on renal function. The mechanism may be through the improvement of impaired lysosomal function.
