目的:系统评价甲钴胺联合α-硫辛酸治疗糖尿病周围神经病变的治疗效果。方法:计算机检索英文数据库Pub Med、Medline、The Cochrane Library和中文数据库中国知网(CNKI)、维普数据库(VIP)、万方数据库等,搜集甲钴胺联合α-硫辛酸(ALA)治疗糖尿病周围神经病变的相关随机对照试验(RCT),检索时限为建库至2015年9月,由2名研究者独立筛选文献、提取相关数据及文献质量评价,应用Re Man 5.2统计软件对纳入的研究数据进行Meta分析。结果:共纳入12篇文献,合计1153例患者,Meta分析结果显示:甲钴胺联合α-硫辛酸可有效提高患者的临床疗效[OR=5.93,95%CI(4.05~8.69),P<0.00001]和超氧化物歧化酶(SOD)[WMD=13.38,95%CI(6.36~20.41),P=0.0002];降低丙二醛(MDA)[WMD=-2.08,95%CI(-2.48~-1.68),P<0.00001]。结论:甲钴胺联合α-硫辛酸治疗糖尿病周围神经病变较单纯使用甲钴胺治疗的效果更显著。
Objective: To systematically evaluate the therapeutic effect of mecobalamin combined with α-lipoic acid in the treatment of diabetic peripheral neuropathy. Methods: Computer-aided search of PubMed, Medline, The Cochrane Library and CNKI, VIP database and Wanfang database were used to collect mecobalamin and alpha-lipoic acid (ALA) for the treatment of diabetes mellitus Neurological disease related randomized controlled trials (RCT), search time limit for the construction of the library until September 2015, by two independent screening of literature, extraction of relevant data and literature quality evaluation, using Re Man 5.2 statistical software for the inclusion of research data Meta-analysis. Results: A total of 12 articles were enrolled in this study. A total of 1153 patients were included in the study. The results of Meta analysis showed that mecobalamin combined with α-lipoic acid could effectively improve the clinical efficacy [OR = 5.93, 95% CI 4.05-8.69, P 0.00001 ], And superoxide dismutase (SOD) [WMD = 13.38, 95% CI 6.36 ~ 20.41, P = 0.0002] 1.68), P <0.00001]. CONCLUSION: Mecobalamin combined with α-lipoic acid is more effective in treating diabetic peripheral neuropathy than mecobalamin alone.
