·病例简介:患者,男,48岁.因拇指背部斑疹4年就诊.斑疹较厚,突出皮肤表面,发痒.rn·可能的诊断:神经性皮炎;汗疱;银屑病;手癣;环状肉芽肿.图1 神经性皮炎rn·诊断:神经性皮炎.rn·讨论:神经性皮炎的诊断主要是根据临床症状,需要与扁平苔藓、银屑病、体癣、接触性皮炎、肉芽肿等进行鉴别.通常情况下,上述这些疾病与神经性皮炎的鉴别较为容易.如果诊断困难,可行活组织检查.病理组织学方面的表现可以有表皮增生、棘层肥厚、真皮层纤维化.在已有皮肤疾病的情况下再发生神经性皮炎的诊断较为困难,例如银屑病患者可以在病变部位再发生神经性皮炎,这种情况下临床及病理学检查可以兼有两种疾病的表现.
