目的:研究十年观察期内种植体支持的固定和可摘义齿修复牙列缺失患者种植体的生存率和影响因素。方法:1995-2006年,227例患者(92例男性,135例女性)平均年龄63.7岁,在Charite’大学口腔颌面外科门诊接受种植和修复治疗。共1652颗种植体种植在牙列缺失上颌骨、下颌骨或双侧颌骨,支持76例(24.68%)固定义齿,232例(75.32%)可摘义齿。观察性别,种植体部位、修复方法对种植体生存率的影响。结果:其中有50例患者73颗种植体丧失,累计种植体生存率94.76%,本研究显示种植体生存率女性(93.93%)略低于男性(95.93%),但没有显著性差异。在上颌骨种植的种植体生存率(95.87%)略高于在下颌骨种植(94.53%),但统计学没有显著性意义。10年累计固定和可摘义齿的种植体生存率分别是98.02%和93.55%,两种修复方法差异有显著性。结论:确认了种植体支持牙列缺失患者修复中长期的成功率,并得出结论种植体生存率与性别,种植部位关系不大,与修复方法有关。种植体支持的固定义齿预后比可摘义齿好。
OBJECTIVE: To study the survival rate and influencing factors of implants implanted with fixed and removable denture supported by implants during the 10-year observation period. METHODS: Between 1995 and 2006, 227 patients (92 males and 135 females) with a mean age of 63.7 years were enrolled in implant and repair treatment at the Charite University Oral and Maxillofacial Surgery Clinic. A total of 1652 implants were implanted in the maxillary, mandible or bilateral maxilla in dentition. They supported 76 (24.68%) fixed dentures and 232 (75.32%) removable dentures. The effects of gender, implant site and repair method on implant survival rate were observed. Results: Of the 50 patients, 73 implants were lost and the cumulative implant survival rate was 94.76%. The study showed that the survival rate of implants was slightly lower than that of males (93.93%), but no significant difference was found. The implant survival rate in the maxilla was slightly higher than that in mandibular implant (95.87%) (94.53%), but the statistics was not statistically significant. The implant survival rates of cumulative fixed and removable teeth in 10 years were 98.02% and 93.55%, respectively. There was significant difference between the two methods. CONCLUSIONS: The long-term success rate of implant restoration in dentition-deficient patients was confirmed and concluded that there was little relationship between implant survival and gender and site of implantation and was associated with repair. Prostheses supported by implants have a better prognosis than removable dentures.
