目的探讨乙酰肝素酶(Heparanase,HPSE)、碱性成纤维细胞生长因子(basic fibroblast growth facto,bF-GF)及组织蛋白酶D(Cathepsin D,Cath-D)与子宫内膜癌浸润、转移的关系。方法采用免疫组化方法检测50例有完整随访资料的子宫内膜癌标本中HPSE、bFGF及Cath-D的表达情况,15例正常子宫内膜和10例非典型增生子宫内膜作为对照,数据用t检验及Spearman秩相关进行分析,总结出结果,找出HPSE、bFGF及Cath-D与子宫内膜癌的关系。结果①HPSE、bFGF及Cath-D从正常子宫内膜、非典型增生子宫内膜到子宫内膜癌三种组织的表达逐渐增加,有统计学差异(PHPSE<0.05,PbFGF<0.05,PCath-D<0.05)。②HPSE、bFGF及Cath-D在手术病理分期II~III期中表达显著高于I期者,有脉管浸润及淋巴转移阳性标本中三种指标均高表达,有统计学差异(均P<0.05)。③HPSE与bFGF表达呈正相关(相关系数r=0.408,P<0.05);Cath-D与bFGF表达呈正相关(相关系数r=0.628,P<0.05)。结论 HPSE、bFGF及Cath-D的表达可能与子宫内膜癌的发生、浸润及转移有密切相关性,三者联合检测对评估子宫内膜癌患者预后有重要作用。
Objective To investigate the effects of heparanase (HPSE), basic fibroblast growth facto (bF-GF) and cathepsin D (Cath-D) on the infiltration and metastasis of endometrial carcinoma relationship. Methods Immunohistochemical method was used to detect the expression of HPSE, bFGF and Cath-D in 50 cases of endometrial carcinoma with complete follow-up data, 15 cases of normal endometrium and 10 cases of atypical hyperplasia endometrium as control, the data T-test and Spearman rank correlation analysis, summarize the results to find the relationship between HPSE, bFGF and Cath-D and endometrial cancer. Results ① The expressions of HPSE, bFGF and Cath-D in normal endometrium, atypical hyperplasia endometrium and endometrial carcinoma increased gradually with statistical significance (PHPSE <0.05, PbFGF <0.05, PCath-D < 0.05). ②The expressions of HPSE, bFGF and Cath-D in stage II-III were significantly higher than those in stage I, and all the three indexes were positive in vascular invasion and lymph node metastasis (all P <0.05) . The positive correlation between HPSE and bFGF expression was found (r = 0.408, P <0.05). Cath-D and bFGF expression were positively correlated (r = 0.628, P <0.05). Conclusion The expression of HPSE, bFGF and Cath-D may be closely related to the occurrence, invasion and metastasis of endometrial carcinoma. The combined detection of these three factors plays an important role in the prognosis of patients with endometrial carcinoma.
