目的观察细胞增殖核抗原Ki-67、血管内皮生长因子(VEGF)和缺氧诱导因子-1a(HIF-1a)蛋白在结直肠腺癌组织中的表达。方法用免疫组化SP法检测32例结直肠腺癌组织中Ki-67、VEGF和HIF-1a蛋白的表达。结果在结直肠腺癌组织中,HIF-1a和VEGF蛋白阳性表达率分别为59.38%(19/32例)和65.63%(21/32例),Ki-67蛋白的阳性表达率为75.0%(24/32例),与癌旁组织比较差异有统计学意义(P<0.01,P<0.05,P<0.05)。Ki-67、VEGF和HIF-1a蛋白表达与结直肠腺癌国际通用的TNM(T:肿瘤大小及局部浸润范围,N:淋巴结受累情况,M:远处转移)分期法中的分期、浸润深度密切相关;而与性别、年龄、肿瘤直径和分化程度无关。Ki-67蛋白表达在浸润深度T3+T4比例为91.30%(21/23例),较浸润深度T1+T2比例33.33%(3/9例),差异有统计学意义(P<0.05);Ki-67蛋白表达在TNM分期Ⅲ+Ⅳ比例为100%(12/12例),较Ⅰ+Ⅱ比例60%(12/20例)升高但差异无统计学意义(P>0.05)。VEGF蛋白表达在浸润深度T3+T4比例为82.60%(19/23例),较浸润深度T1+T2比例22.22%(2/9例)明显升高,差异有统计学意义(P<0.05);VEGF蛋白表达在TNM分期Ⅲ+Ⅳ比例为91.67%(11/12例),较Ⅰ+Ⅱ比例50%(10/20例)明显升高,差异有统计学意义(P<0.05)。HIF-1a蛋白表达在浸润深度T3+T4比例为73.91%(17/23例),较浸润深度T1+T2比例22.22%(2/9例)明显升高,差异有统计学意义(P<0.05);HIF-1a蛋白表达在TNM分期Ⅲ+Ⅳ比例为91.67%(11/12例),较Ⅰ+Ⅱ比例40%(8/20例)明显升高,差异有统计学意义(P<0.05)。癌组织中的Ki-67、VEGF和HIF-1a蛋白表达,三者呈正相关,差异有统计学意义(P<0.05)。结论 Ki-67、VEGF和HIF-1a蛋白在癌组织中表达,其可能与结直肠腺癌的发生、发展及淋巴结转移有关。
Objective To investigate the expression of proliferating cell nuclear antigen Ki-67, vascular endothelial growth factor (VEGF) and hypoxia-inducible factor-1a (HIF-1a) in colorectal adenocarcinoma. Methods The expressions of Ki-67, VEGF and HIF-1a in 32 cases of colorectal adenocarcinoma were detected by immunohistochemical SP method. Results The positive expression rates of HIF-1a and VEGF in colorectal adenocarcinoma were 59.38% (19/32 cases) and 65.63% (21/32 cases), respectively. The positive expression rate of Ki-67 protein was 75.0% 24/32 cases). The difference between them was statistically significant (P <0.01, P <0.05, P <0.05). The expression of Ki-67, VEGF and HIF-1a in colorectal adenocarcinoma is closely related to the TNM stage (TN: tumor size and local infiltration range, N: lymph node involvement, M: distant metastasis) Closely related; but not with gender, age, tumor diameter and degree of differentiation. The expression of Ki-67 protein was 91.30% (21/23) in the depth of invasion T3 + T4, 33.33% (3/9) higher than the depth of invasion T1 + T2 (P <0.05). The Ki The expression of -67 protein in TNM stage Ⅲ + Ⅳ was 100% (12/12 cases), which was higher than that in Ⅰ + Ⅱ 60% (12/20 cases), but the difference was not statistically significant (P> 0.05). The expression of VEGF protein in the depth of invasion T3 + T4 was 82.60% (19/23), which was significantly higher than that in the depth of invasion T2 + 22.22% (2/9), the difference was statistically significant (P <0.05). The expression of VEGF protein in TNM stage Ⅲ + Ⅳ was 91.67% (11/12 cases), which was significantly higher than that in Ⅰ + Ⅱ 50/20 cases (10/20 cases), the difference was statistically significant (P <0.05). The expression of HIF-1a protein in the depth of invasion T3 + T4 was 73.91% (17/23), which was significantly higher than that in the depth of invasion T2 + 22.22% (2/9) ). The expression of HIF-1a protein in TNM stage Ⅲ + Ⅳ was 91.67% (11/12 cases), which was significantly higher than that in case Ⅰ + Ⅱ (8/20 cases), the difference was statistically significant (P <0.05 ). There was a positive correlation between the expression of Ki-67, VEGF and HIF-1a in cancer tissues (P <0.05). Conclusion The expression of Ki-67, VEGF and HIF-1a protein in cancer tissues may be related to the occurrence and development of colorectal adenocarcinoma and lymph node metastasis.
