目的:探讨早期卧位踏车训练对亚急性期脑卒中患者功能预后的影响。方法:将符合入选标准的脑卒中偏瘫患者随机分为对照组(20例)和治疗组(20例),对照组患者采用常规康复训练方法如神经肌肉促进技术、功能性电刺激、直立床训练、肌电生物反馈及针灸等,治疗组在此基础上,增加卧位踏车训练,使用德国Reck Motomed letto2型智能运动训练系统。治疗前和治疗2~4周后分别采用Berg平衡量表(Bergbalance scale,BBS)、Fugl-Meyer量表(Fugl-Meyer assessment,FMA)及Barthel指数(the Barthel index of ADL,BI)对患者的平衡、下肢运动及日常生活活动能力进行评价。结果:2组患者的平衡、运动、步行和ADL能力治疗后均有改善(p<0.05),但治疗组患者的效果优于对照组(p<0.05)。结论:早期卧位踏车活动能有效改善及提高亚急性期脑卒中患者的功能,预后良好。
Objective: To investigate the effect of early treadmill training on the prognosis of patients with subacute stroke. Methods: The patients with hemiplegia were randomly divided into control group (n = 20) and treatment group (n = 20). The patients in control group were given routine rehabilitation training such as neuromuscular stimulation, functional electrical stimulation, vertical bed training , EMG biofeedback and acupuncture and so on. On the basis of this, the treatment group increased the training of lying-position treadmill and used the German Reck Motomed letto2-type intelligent exercise training system. Before treatment and after 2 to 4 weeks of treatment, patients were assessed with the Bergbalance scale (BBS), the Fugl-Meyer assessment (FMA) and the Barthel index of ADL (BI) Balance, lower extremity movements and activities of daily living evaluation. Results: The balance, exercise, walking and ADL abilities in both groups improved after treatment (p <0.05), but the effect of the treatment group was better than that of the control group (p <0.05). Conclusion: The treadmill exercise in early lying position can effectively improve and enhance the function of stroke patients in subacute stage, and the prognosis is good.
