目的分析原发性痛经患者经期前后子宫动脉血流动力学的彩色多普勒超声改变,并探讨与虚实辨证之间的关系。方法选取40例在校女大学生原发性痛经者和30例正常对照者,对痛经组按中医辨证分型分为实证组与虚证组,在月经前、行经早期和行经后期行彩色多普勒超声检查,测量子宫动脉及各分支的阻力指数值(RI)。结果与对照组比较,实证组月经前后弓状动脉、放射动脉、螺旋动脉RI增高;虚证组经期各参数较对照组增高;各组不同时期自身对比,对照组行经后期各参数较经前期高,实证组经期前后各参数比较无显著差异,虚证组经期各参数均高于经前期。结论经腹彩色多普勒超声可以用于评价原发性痛经子宫血流动力学的改变,并可望有助于原发性痛经的中医证分型。
Objective To analyze the color Doppler ultrasound changes of uterine artery hemodynamics in patients with primary dysmenorrhea before and after menstruation, and to explore the relationship between syndrome differentiation and deficiency and weakness. Methods 40 cases of female students with primary dysmenorrhea in school and 30 cases of normal control group, the dysmenorrhea group according to TCM syndrome differentiation is divided into empirical group and deficiency syndrome group, before menstruation, early and late through color Doppler Ultrasonography was performed to measure the resistance index (RI) of the uterine artery and its branches. Results Compared with the control group, the posterior arcuate, radiating artery and spiral artery RI in the experimental group were higher than those in the control group. The parameters of asthenia syndrome in the menstrual period were higher than those in the control group. Compared with the control group, There was no significant difference in the parameters of the empirical group before and after menstruation. The parameters of the asthenia syndrome group were all higher than the premenstrual ones. Conclusion Transabdominal color Doppler ultrasound can be used to evaluate the changes of hemodynamics in primary dysmenorrhea uterus, and is expected to contribute to the TCM syndrome type of primary dysmenorrhea.
