目的探讨微量白蛋白尿(MAU)与慢性心力衰竭(CHF)患者心功能之间的关系。方法对2015年1月-2016年1月期间在武清区人民医院接受治疗的100例出现蛋白尿的CHF患者的病历资料进行回顾性研究。根据患者心功能的不同将其分为心功能Ⅱ级组(33例)、心功能Ⅲ级组(39例)和心功能IV级组(28例)。同时选取同一时期在武清区人民医院体检正常者40例为对照组。测定并比较4组的MAU、脑钠肽(BNP),另对心功能分级与MAU进行相关性分析。结果 BNP水平比较心功能Ⅳ级组>Ⅲ级组>Ⅱ级组>对照组差异有统计学意义(P<0.05)尿液中白蛋白的含量比较,心功能Ⅳ级组>Ⅲ级组>Ⅱ级组>对照组差异有统计学意义(P<0.05)。MAU水平与CHF患者的心功能分级具有相关性(r=0.59,P<0.05)。结论 CHF患者可出现白蛋白尿的情况而且MAU与CHF的严重程度密切相关。
Objective To investigate the relationship between microalbuminuria (MAU) and cardiac function in patients with chronic heart failure (CHF). Methods The clinical data of 100 patients with proteinuria in CHF who were treated in Wuqing District People’s Hospital from January 2015 to January 2016 were retrospectively reviewed. According to their cardiac function, they were divided into two groups: heart function Ⅱ group (33 cases), heart function Ⅲ group (39 cases) and heart function class Ⅳ group (28 cases). At the same time select the same period in Wuqing District People’s Hospital, 40 cases of normal physical examination as a control group. MAU and brain natriuretic peptide (BNP) in 4 groups were determined and compared. Correlation analysis between MAU and other grading of cardiac function was performed. Results There was significant difference in serum BNP level Ⅳ> Ⅲ> Ⅱ> control group (P <0.05), albumin level in urine, Level group> control group, the difference was statistically significant (P <0.05). There was a correlation between MAU level and heart function grading in CHF patients (r = 0.59, P <0.05). Conclusion There may be albuminuria in CHF patients and MAU is closely related to the severity of CHF.
