目的调查上海市中老年糖调节受损(IGR)人群胰岛β细胞功能并对相关因素进行分析。方法对大宁路街道的588名小区居民进行糖耐量筛查,测定生化指标,并进行统计分析。结果共检出IGR 90例(15.3%),其中单纯空腹血糖受损(IFG)9例(10%)、单纯糖耐量减低(IGT)66例(73.3%)、IFG+IGT 15例(16.7%)。对资料完整的60例患者按年龄分为A组(45~49岁)、B组(50~59岁)、C组(60~69岁)、D组(≥70岁)。A、B组FIns、2hIns水平高于C、D组。A、B组胰岛素抵抗指数(HOMA-IR)、胰岛素分泌功能指数(HOMA-β)高于C、D组。年龄与FIns、2hIns水平负相关。A、B组血清高敏C反应蛋白(hsC-RP)水平低于C、D组。结论随着年龄的增长,胰岛β细胞功能逐渐减退,导致糖耐量减低发生;亚临床炎症可能是其发生机制之一。
Objective To investigate the function of pancreatic islet β cells in people with impaired glucose regulation (IGR) in Shanghai and analyze the related factors. Methods 588 community residents in Daning Road were screened for glucose tolerance and their biochemical parameters were determined and statistically analyzed. Results A total of 90 IGR cases (15.3%) were detected, including 9 cases (10%) with impaired fasting glucose (IFG), 66 cases with simple glucose tolerance (IGT) ). Sixty patients with complete data were divided into A group (45-49 years old), B group (50-59 years old), C group (60-69 years old) and D group (70 years old) by age. A, B group FIns, 2hIns level was higher than C, D group. The insulin resistance index (HOMA-IR) and insulin secretion index (HOMA-β) of group A and group B were higher than those of group C and D. Age and FIns, 2hIns levels were negatively correlated. Serum high-sensitivity C-reactive protein (hsC-RP) levels in group A and B were lower than those in group C and D. Conclusion With the increase of age, the function of islet β cells gradually diminishes, resulting in impaired glucose tolerance; subclinical inflammation may be one of the mechanisms.
