目的观察高压氧(HBO)对缺血性脑卒中血浆D-二聚体、纤维蛋白原(Fg)含量的影响。方法选取常规治疗组97例、HBO治疗组137例及正常对照组40例,在入院第2,10,20天对其血浆D-二聚体、Fg水平用STAGO公司血凝仪及配套试剂盒进行检测。结果常规治疗组血浆D-二聚体、Fg水平入院时比正常对照组升高(P<0.01或<0.05);第10天血浆D-二聚体持续升高(P<0.01),Fg水平低于正常对照组(P<0.01);第20天血浆D-二聚体水平下降但仍高于正常对照组(P<0.05),Fg水平恢复正常,与正常对照组差异无统计学意义(P>0.05)。HBO治疗组血浆D-二聚体、Fg水平入院时比对照组升高(P<0.01或<0.05);第10天血浆D-二聚体持续升高(P<0.01)但低于常规治疗组(P<0.05),Fg水平下降但高于正常对照组(P<0.05);第20天血浆D-二聚体、Fg水平与正常对照组差异无统计学意义(P>0.05)。结论HBO能调节D-二聚体、Fg水平,可减轻因缺血、血管损伤引起的凝血、抗凝及纤溶系统异常,对缺血性脑卒中的治疗、预防复发有显著效果。
Objective To observe the effects of hyperbaric oxygen (HBO) on plasma D-dimer and fibrinogen (Fg) in ischemic stroke. Methods 97 cases of routine treatment group, 137 cases of HBO treatment group and 40 cases of normal control group, on the 2nd, 10th, 20th day of admission, the plasma D-dimer and Fg level with STAGO coagulator and kit Test. Results The levels of plasma D-dimer and Fg in patients in the conventional treatment group were significantly higher than those in the normal control group (P <0.01 or <0.05), and the levels of plasma D-dimer were increased (P <0.01) (P <0.01). On the 20th day, plasma D-dimer level was still lower than that of the normal control group (P <0.05), and Fg level returned to normal. There was no significant difference between the normal control group and the normal control group P> 0.05). The levels of plasma D-dimer and Fg in HBO group were higher than those in control group at admission (P <0.01 or <0.05), and the plasma D-dimer level increased continuously (P <0.01) (P <0.05). The level of Fg decreased but was higher than that of the normal control group (P <0.05). On the 20th day, the plasma levels of D-dimer and Fg were not significantly different from those of the normal control group (P> 0.05). Conclusion HBO can regulate D-dimer and Fg levels, which can reduce the abnormalities of coagulation, anticoagulation and fibrinolysis system caused by ischemia and vascular injury, and have a significant effect on the treatment and prevention of ischemic stroke.
