16例急性早幼粒细胞白血病患者血清TNF活性明显高于正常对照(P<0.05),而经RA诱导分化治疗中期及治疗后明显降低(P<0.05),但仍高于正常对照组。各治疗时相的外周白细胞胞浆内TNF平均水平及正常对照间均无明显差别(P>0.05),治疗前及治疗中期胞浆TNF活性升高率明显高于治疗后或正常对照(P<0.05)。相关分析表明:血清TNF活性水平与外周血WBC总数或骨髓GM-CFU无明显相关,而与骨髓L-CFU变化具相关性(P<0.01)。体外rhTNF抑制正常造血,RA轻度促进造血,ATRA+TNF可拮抗TNF对正常造血的抑制。RA、TNF或RA+TNF均明显抑制NB_4细胞株体外增殖(P<0.05)。
Serum TNF activity in 16 patients with acute promyelocytic leukemia was significantly higher than that in the normal control (P <0.05), while RA-induced differentiation was significantly lower (P <0.05) and higher than that in the normal control group. There was no significant difference between the mean levels of TNF in peripheral leukocytes and normal controls at each treatment phase (P> 0.05). The increase of cytosolic TNF activity before and during the treatment was significantly higher than that after treatment or normal control (P < 0.05). Correlation analysis showed that the level of serum TNF activity was not significantly correlated with the total number of peripheral blood WBC or bone marrow GM-CFU, but with the change of L-CFU of bone marrow (P <0.01). In vitro rhTNF inhibits normal hematopoiesis and RA promotes hematopoiesis. ATRA + TNF antagonizes the inhibition of TNF on normal hematopoiesis. RA, TNF or RA + TNF significantly inhibited the proliferation of NB4 cell line in vitro (P <0.05).
