目的:探索新生儿多脏器功能衰竭(MSOF)的病因及其转归,提高对MSOF的抢救水平,降低新生儿病死率。方法:时1994~1998 年新生儿重症监护室住院的176例MSOF患儿的病因进行分析。结果:在窒息、败血症、呼吸窘迫综合征、肺炎、先天性心脏病、寒冷损伤及溶血病等主要原发病所致的MSOF中,除先天性心脏病外,均以呼吸衰竭发生率最高。脑功能衰竭于窒息及溶血时发生率较高,肾功能衰竭于寒冷损伤、败血症及溶血病时较高。结论:积极治疗原发病,防治及终止各种侵袭的延续,维持内外环境的平衡和及时给予呼吸支持是成功治疗MSOF的关键。
Objective: To explore the etiology and prognosis of neonatal multiple organ failure (MSOF), improve the rescue of MSOF and reduce the neonatal mortality. Methods: The etiology of 176 MSOF children hospitalized in neonatal intensive care units from 1994 to 1998 was analyzed. Results: In the MSOF caused by the main primary diseases of asphyxia, sepsis, respiratory distress syndrome, pneumonia, congenital heart disease, cold injury and hemolytic disease, the incidence of respiratory failure was the highest except for congenital heart disease. Brain failure occurs asphyxia and hemolysis when the higher incidence of renal failure in cold injury, sepsis and hemolytic disease is higher. Conclusion: The key to successful treatment of MSOF is to actively treat the primary disease, prevent and stop the continuation of all kinds of invasion, maintain the balance of internal and external environment and timely give respiratory support.
