目的观察胸胃减容对食管癌术后早期并发症的预防作用。方法自2003年4月至2005年4月同期对比30例胸胃减容组和30例传统手术组的术后早期并发症的发生情况,心肺功能的改变。结果减容组术后发生肺炎1例,对照组发生膈疝1例,肺不张3例,胃瘫1例。术后3日晨停吸氧30分钟血气分析平均氧分压较术前均有下降,对比有显著差异;术后第14天肺功能检查同术前百分比,减容组对照组对比有差异。结论胸胃减容是一种安全、有效、实用的减少食管癌术后早期并发症的预防方法。
Objective To observe the preventive effect of thoracic gastric volume reduction on postoperative complications of esophageal cancer. Methods From April 2003 to April 2005, the incidence of early postoperative complications and the changes of cardiopulmonary function were compared between 30 cases of thoracic stomach volume reduction group and 30 cases of traditional operation group. Results One case of postoperative pneumonia occurred in the volume reduction group, while in the control group, there was 1 case of sputum, 3 cases of pulmonary atelectasis, and 1 case of gastric fistula. On the 3rd day after surgery, the blood oxygen analysis showed that the average oxygen partial pressure decreased compared with that before surgery. There was a significant difference in the comparison between the two groups. The percentage of lung function tests on the 14th day after surgery was the same as the preoperative percentage and the volume reduction group. Conclusions thoracic gastric volume reduction is a safe, effective and practical method to reduce the early complications of esophageal cancer surgery.
