目的 探讨高龄贲门癌的临床特点及采用管型吻合器经腹施行贲门癌根治术的临床价值。方法 回顾分析 1991～2 0 0 1年收治的 80例高龄贲门癌患者吻合器的治疗情况。结果 本组平均发病年龄 70岁 ,男女发病比例为 5∶1,平均病程 3 2个月。早期诊断率为 2 5 %。术前并存疾病的发生率为 60 %。应用吻合器经腹外科治疗 ,根治切除率 72 5 % ,姑息切除率 18 8% ,术后并发症发生率为 5 0 % ,住院院死亡率 1 2 5 % ,后两者明显低于胸腹联合手术 ,切除的 5年生存率为 2 0 %。结论 经腹部采用管腔式吻合器施行高龄贲门癌根治术 ,对患者呼吸循环系统影响小 ,可以降低手术风险 ,减少并发症。
Objective To investigate the clinical features of advanced cardia cancer and the clinical application of tubular stapler for ablation of cardia cancer. Methods Retrospective analysis of 80 cases of elderly patients with cardia cancer stapled surgery in 1991 ~ 2001 treatment. Results The average age of onset of this group of 70 years old, male and female incidence ratio of 5: 1, the average duration of 32 months. The early diagnosis rate was 25%. Preoperative co-morbidity was 60%. Application of stapler surgery by abdominal surgery, radical resection rate of 72.5%, palliative resection rate of 18.8%, postoperative complications was 50%, hospitalized hospitalized mortality rate of 125%, the latter two were significantly lower than the thoracoabdominal Combined surgery, resection of the 5-year survival rate was 20%. Conclusions Transluminal lumbar stapler operation for the treatment of advanced gastric cardia carcinoma has a little effect on the respiratory system of the patients, which can reduce the operation risk and reduce the complications.
