目的探讨PTEN、p53及hMSH2在子宫内膜癌中的蛋白表达及与病理分级的关系。方法用免疫组织化学SP法检测大连医科大学第二临床学院和抚顺市中心医院妇产科1999年7月至2004年9月收治的99例(病理分级Ⅰ级59例、Ⅱ和Ⅲ级40例)子宫内膜癌中PTEN、p53与hMSH2的蛋白表达。结果子宫内膜癌组织PTEN蛋白表达总缺失率70·7%,病理Ⅰ级为81·4%,Ⅱ和Ⅲ级55·0%,二者呈负相关(P<0·05);p53蛋白阳性表达率42·4%,Ⅰ级20·3%,Ⅱ和Ⅲ级75·0%,二者差异有显著性意义(P<0·01);hMSH2蛋白阳性表达率60·6%,Ⅰ级57·6%,Ⅱ和Ⅲ级65·0%,二者无明显相关性(P>0·05)。在hMSH2(+)中PTEN蛋白表达缺失率57·1%,hMSH2(-)42·9%;在hMSH2(+)中p53蛋白表达阳性率78·6%,hMSH2(-)21·4%。PTEN与hMSH2无相关性(P>0·05)。p53与hMSH2呈正相关(P<0·05)。结论检测PTEN、p53蛋白表达可能有助于判断子宫内膜癌的程度。p53基因突变与错配修复基因的功能缺陷有关。
Objective To investigate the protein expression of PTEN, p53 and hMSH2 in endometrial carcinoma and its relationship with pathological grade. Methods Immunohistochemical SP method was used to detect 99 cases of pathological grade Ⅰ in 59 cases, Ⅱ and Ⅲ in 40 cases from July 1999 to September 2004 in Department of Obstetrics and Gynecology, Dalian Second Medical University and Fushun Central Hospital. ) Protein expression of PTEN, p53 and hMSH2 in endometrial cancer. Results The total deletion rate of PTEN protein in endometrial carcinoma was 70.7%, the pathological grade Ⅰ was 81.4%, and the grade Ⅱ and Ⅲ were 55.0% (P <0.05) The positive expression rate of hMSH2 protein was 42.4%, Ⅰ grade was 20.3%, Ⅱ and Ⅲ were 75.0%, the difference was significant (P <0.01). The positive expression rate of hMSH2 protein was 60.6% Grade 57.6%, Grade II and Grade III 65.0%, there was no significant correlation between them (P> 0.05). The positive rate of p53 protein in hMSH2 (+) was 78.6% and that of hMSH2 (-) was 21.4% in hMSH2 (+). There was no correlation between PTEN and hMSH2 (P> 0.05). p53 was positively correlated with hMSH2 (P <0.05). Conclusion Detection of PTEN and p53 protein expression may be helpful in judging the degree of endometrial cancer. p53 gene mutations and mismatch repair genes related to functional defects.
