目的探讨轻度老年性痴呆(Alzhei mer’s disease,AD)患者听功能与神经心理量表测评的关系。方法测试50例正常老年人(对照组)和15例轻度AD患者(AD组)的周围听觉功能(纯音听阈、言语识别率)以及中枢听觉功能(同侧竞争信息-合成句识别试验、对侧竞争信息-合成句识别试验、交错扬扬格词试验、掩蔽级差试验),将各项神经心理量表分为言语相关性和非言语相关性两部分进行测评,分析测听结果和量表得分,比较听觉功能和神经心理量表结果之间的关系。结果AD患者周围听觉功能(纯音听阈、言语识别率)同言语相关性认知量表的关系(Kendall’s tau b分别为-0.33和0.44)比其与非言语相关性认知量表的关系(Kendall’s tau b分别为-0.23和0.26)更明显(P<0.05),对照组中无此现象。轻度AD患者中枢听觉功能(同侧竞争信息-合成句识别试验、对侧竞争信息-合成句识别试验、交错扬扬格词试验、掩蔽级差试验)与言语相关性和非言语相关性认知量表均有明显相关性(Kendall’s tau b范围0.37~0.56),与周围听功能与言语相关性和非言语相关性认知量表关系相比较,差异有统计学意义(P<0.05)。结论轻度AD患者的周围听功能障碍与言语相关性认知量表测评结果有关,其中枢听功能障碍与言语相关性和非言语相关性认知量表测评结果均有关,应用神经心理学量表测评轻度AD患者认知功能时,应注意听功能障碍对其结果的影响。
Objective To investigate the relationship between auditory function and neuropsychological assessment in patients with mild Alzheimer’s disease (AD). Methods Peripheral auditory function (pure tone threshold, speech recognition rate) and central auditory function in 50 normal controls (AD group) and 15 mild AD patients (AD group) were tested. Central auditory function in mild AD patients (ipsilateral competition information - compound sentence recognition test, contralateral competitive information - compound sentence recognition test, staggered upright word test, masking grade test) verbal relevance and nonverbal relevance (Kendall’s tau b range: 0.37-0.56). The difference was statistically significant (P <0.05) compared with the correlation between cognitive function and speech-related and non-speech-related cognitive scales. Conclusion Peripheral auditory dysfunction in patients with mild AD is related to the verbal cognitive cognitive assessment scale.
