目的:探讨异丙酚复合麻醉下行鼻息肉摘除术对患者纤溶功能的影响作用。方法采用随机数字表法将100例择期行鼻息肉摘除手术的患者分为观察组(异丙酚复合麻醉)和对照组(七氟醚复合麻醉),每组50例,对比两组患者不同时刻纤溶功能等指标的变化。结果观察组的麻醉效果优良率为96%,高于对照组的92%,但差异无统计学意义(P>0.05);观察组与对照组的患者在各个时刻的心率(HR)、呼吸频率(RR)、平均动脉压(MAP)测定值差异均无统计学意义(P>0.05);观察组和对照组患者在T1时刻的血浆D二聚体(DD)、组织型纤溶酶原激活物(t-PA)、纤溶酶原激活物抑制物(PAI-1)水平差异无统计学意义(P>0.05);观察组的DD、t-PA水平在T2、T3时刻显著高于对照组(P<0.05),PAI-1水平显著低于对照组(P<0.05),差异有统计学意义。结论异丙酚复合麻醉与七氟醚复合麻醉下行鼻息肉摘除术的麻醉效果相当,但前者具有较强的增强患者的纤溶功能,更适宜于进行鼻息肉摘除手术。“,”Objective The purpose of this study was to investigate the effects of propofol combined with sevoflurane anesthesia on fibrinolytic function in patients with nasal polypectomy. Methods 100 cases of patients receiving nasal polypectomy in our otolaryngology department from 2011 July to 2014 December were divided into observation group (propofol) and control group (sevoflurane) 50 cases in each group using random number table method. The fibrinolytic function index changes in the two groups were compared. Results The anesthesia effect of the observation group (96%) was higher than that of the control group (92%), but the difference was not statistically significant (P>0.05). There were no significant differences in HR, MAP and RR between the observation group and the control group (P>0.05). There were no significant differences in DD, t-PA and PAI-1 between the observation group and the control group (P>0.05). The levels of DD and t-PA in the observation group at T2 and T3 time points were significantly higher than those in the control group, and PAI-1 levels were significantly lower than those in the control group (P<0.05). Conclusion Combined propofol and sevoflurane anesthesia for nasal polypectomy showed similar anesthesia effect to sevoflurance alone , but it could enhance fiber dissolves the function, and was more suitable for nasal polypectompy.
