目的 :观察异丙酚与芬太尼联合持续静脉输注用于 ICU镇静的作用。方法 :对 30例腹部手术后机械通气病人给予异丙酚与芬太尼联合 (10 0∶ 0 .1)持续静脉输注。结果 :总镇静时间为 (2 3.3± 11.7) h,其中满意镇静时间占 (90 .1± 8.3) % ,异丙酚总平均静注速度为 (6 .5± 3.1) μg· kg- 1· m in- 1 ,满意镇静期间的平均静注速度为(5 .5± 2 .7) μg· kg- 1· m in- 1 ,镇静期间心率平稳 ,血压略降但维持稳定。停止镇静后病人完全清醒时间为 0~ 2 0min,脱离呼吸机时间为 (19.5± 16 .9) m in,拔气管导管时间为 (6 6 .3± 2 3.3) min,拔气管导管后 30 min血气正常。结论 :芬太尼与异丙酚联合用于 ICU病人镇静效果满意 ,并可以明显减少异丙酚用量。
Objective: To observe the effect of propofol and fentanyl combined with continuous intravenous infusion on ICU sedation. Methods: Thirty patients with mechanical ventilation after abdominal surgery were given continuous infusion of propofol and fentanyl (100: 0.1). Results: The total sedation time was (22.3 ± 11.7) h, of which the satisfactory sedation time was (90.1 ± 8.3)% and the total intravenous propofol infusion rate was (6.5 ± 3.1) μg · kg-1 · m-1. The mean intravenous infusion rate during satisfactory sedation was (5.5 ± 2 .7) μg · kg-1 · m-1. During the sedation, heart rate was stable and blood pressure was slightly decreased but remained stable. After stopping sedation, the patient’s complete awake time was 0 to 20 minutes, the time from the ventilator was (19.5 ± 16.9) minutes, the time of tracheal intubation was (6.63 ± 2.33) min, and the time of tracheal extubation was 30 minutes Blood gas is normal. Conclusions: The combination of fentanyl and propofol in patients with ICU sedation is satisfactory, and can significantly reduce the amount of propofol.
