目的:提高对甲状旁腺功能减退症(甲旁减)低钙血症致继发性癫痫的诊疗认识。方法:分析1例甲旁减致继发性癫痫患者的临床资料,并结合文献进行讨论。结果:甲旁减患者术后长期低钙血症致全身多器官功能受损,其中脑细胞功能受损后易表现为脑基底节区钙化而引起继发性癫痫发作,这种罕见的继发性癫痫容易造成临床误诊,积极补钙治疗可以预防癫痫复发,无须常规抗癫痫治疗。结论:甲旁减致继发性癫痫罕见,临床表现为较为规律的癫痫发作,维持血钙正常是预防继发性癫痫的主要治疗手段。
OBJECTIVE: To improve the diagnosis and treatment of secondary epilepsy caused by hypocalcemia in patients with hypoparathyroidism. Methods: The clinical data of one case of paraparoxysmal secondary epilepsy were analyzed and the literature was discussed. RESULTS: Long-term hypocalcemia resulted in impaired systemic multiple organ function in patients with hypoparathyroidism, and brain cell dysfunction was likely to result in calcification of the basal ganglia and secondary epileptic seizures. This rare secondary Epilepsy can easily lead to clinical misdiagnosis. Active calcium supplementation can prevent epilepsy recurrence without conventional antiepileptic treatment. Conclusion: Parathyroid degeneration is rare in secondary epilepsy. The clinical manifestations are more regular epileptic seizures, and maintenance of normal serum calcium is the main treatment to prevent secondary epilepsy.
