目的探讨上皮型钙黏素E(E-cad)、细胞周期素D1(Cyclin D1)和β-连环蛋白(β-cat)与胃癌发生及胃癌生物学行为的关系。方法应用免疫组化法检测E-cad、Cyclin D1和β-cat在50例胃癌组织标本、10例正常胃黏膜标本及20例胃癌前病变组织标本中的表达。结果胃癌前病变的E-cad、Cyclin D1和β-cat异常表达率分别为50%、60%、60%,分别与胃癌E-cad(84.0%)、Cyclin D1(72.0%)、β-cat(78.0%)的异常表达率相比差异有统计学意义(P<0.05)。低分化胃癌的E-cad、Cyclin D1和β-cat异常表达率分别为80%、90%、73.4%,分别比高分化胃癌E-cad(50%)、Cyclin D1(55%)、β-cat(40%)的异常表达更明显(P<0.05)。胃癌浸润到浆膜外及伴淋巴结转移时E-cad的异常表达率较高(P<0.01)。肿瘤直径≥5.0cm的胃癌组E-cad的异常表达率较高(P<0.05)。90%的胃癌及60%的胃癌前病变E-cad系统中至少有一项表达异常,二者差异有统计学意义(P<0.05),且至少有一项为表达异常患者的淋巴结转移率明显高于无蛋白异常表达者(P<0.05)。结论E-cad和β-cat的异常表达与胃癌的分化和转移密切相关,β-cat异常表达影响Cyclin D1的激活而引起细胞增殖和分化失控。E-cad、Cyclin D1和β-cat在由胃不典型增生转化为胃癌的过程中起作用。联合检测E-cad、β-cat和Cyclin D1可提高检测结果的参考价值。E-cad、β-cat和Cyclin D1可作为反映胃癌生物学行为的指标。
Objective To investigate the relationship between the expression of epithelial cadherin E (E-cad), cyclin D1 (β-catenin) and gastric carcinogenesis and the biological behavior of gastric cancer. Methods The expressions of E-cad, Cyclin D1 and β-cat were detected by immunohistochemistry in 50 specimens of gastric cancer tissues, 10 specimens of normal gastric mucosa and 20 specimens of gastric precancerous lesions. Results The abnormal expression rates of E-cad, Cyclin D1 and β-cat in gastric precancerous lesions were 50%, 60% and 60% (78.0%), the difference was statistically significant (P <0.05). The poorly differentiated gastric cancer E-cad, Cyclin D1 and β-cat abnormal expression rates were 80%, 90%, 73.4%, respectively, than the well-differentiated gastric cancer E-cad (50% The abnormal expression of cat (40%) was more obvious (P <0.05). The abnormal expression rate of E-cad in gastric cancer infiltrating to the outside of the serosa and with lymph node metastasis was higher (P <0.01). The abnormal expression rate of E-cad in gastric cancer group with tumor diameter≥5.0cm was higher (P <0.05). 90% of gastric cancer and 60% of gastric precancerous lesions E-cad system at least one abnormal expression, the difference was statistically significant (P <0.05), and at least one of the abnormal expression of lymph node metastasis was significantly higher No abnormal protein expression (P <0.05). Conclusion The abnormal expression of E-cad and β-cat is closely related to the differentiation and metastasis of gastric cancer. The abnormal expression of β-cat affects the activation of Cyclin D1 and causes uncontrolled cell proliferation and differentiation. E-cad, Cyclin D1 and β-cat play a role in the process of gastric dysplasia into gastric cancer. Joint detection of E-cad, β-cat and Cyclin D1 can improve the reference value of test results. E-cad, β-cat and Cyclin D1 can be used as indicators of biological behavior of gastric cancer.
