目的:探讨丹参多酚酸盐、压力袜联合低分子肝素钙预防妇科术后下肢深静脉血栓的临床效果。方法:选取2016年1月至2017年1月东莞市长安医院100例妇科术后患者作为研究对象,按照预防方法分为观察组和对照组,各50例。对照组给予压力袜联合低分子肝素钙,观察组在此基础上加用丹参多酚酸盐,比较两组患者术前、术后1 d、术后3 d及术后5 d的各项凝血指标(血小板计数、凝血酶原时间、纤维蛋白原、D–二聚体)检测值;比较两组患者术前及术后1 d、术后3 d及术后7 d股静脉血流峰速度和平均速度;比较两组下肢深静脉血栓形成(DVT)发生率。结果:术后1 d、3 d及5 d研究组血小板计数均低于对照组,纤维蛋白原均低于对照组,D–二聚体均低于对照组,凝血酶原时间均高于对照组,各项指标组间比较,差异均具有统计学意义(P<0.05)。术后1 d、术后3 d及术后7 d观察组股静脉血流峰速度和平均速度均大于对照组,差异具有统计学意义(P<0.05)。观察组术后DVT发生率低于对照组,差异具有统计学意义(P<0.05)。结论:丹参多酚酸盐、压力袜联合低分子肝素钙预防妇科术后下肢深静脉血栓的效果更好。
Objective: To investigate the clinical effect of Salviae miltiorrhizae polyphenols and pressure socks combined with low-molecular-weight heparin on the prevention of lower extremity deep vein thrombosis after gynecological operation. Methods: A total of 100 gynecological patients from Chang’an Hospital of Dongguan City from January 2016 to January 2017 were selected as study subjects and divided into observation group and control group according to the prevention methods, 50 cases each. The control group was given pressure socks combined with low molecular weight heparin, and the observation group was given Salviae miltiorrhizae polyphenolate on the basis of the above. The coagulation of each group was compared before operation, 1 d after operation, 3 d after operation and 5 d after operation (Platelet count, prothrombin time, fibrinogen, D-dimer) were measured before and after operation. The blood flow velocity of femoral vein at 1 d, 3 d and 7 d after operation were compared between the two groups And average speed. The incidence of deep venous thrombosis (DVT) in both groups was compared. Results: The platelet counts of the study group were lower than those of the control group on the 1st, 3rd, and 5th day after surgery, while the fibrinogen was lower than that of the control group, while the D-dimer was lower than that of the control group. The prothrombin time was higher than that of the control group Group, the indicators between the groups were compared, the differences were statistically significant (P <0.05). The peak flow velocity and average velocity of femoral vein at 1 d after operation, 3 d after operation and 7 d after operation were all higher than those in control group, with statistical significance (P <0.05). The incidence of postoperative DVT in the observation group was lower than that in the control group, the difference was statistically significant (P <0.05). Conclusion: Salviae miltiorrhizae polyphenols and pressure socks combined with low molecular weight heparin calcium are effective in preventing postoperative lower extremity deep vein thrombosis.
