目的探讨原发性高血压患者脉压与颈动脉功能改变的相关性。方法选取原发性高血压患者98例,根据动态血压监测结果将患者分为脉压﹥60mmHg组及≤60mmHg组,应用高频超声测量颈动脉僵硬度、颈动脉扩张性。结果脉压﹥60mmHg组颈动脉僵硬度为(1437.33±497.41)mmHg,显著高于脉压≤60mmHg组的(843.46±214.72)mmHg(P<0.05),颈动脉扩张性显著降低[(0.21±0.11)×10-2mmHg-1vs(0.39±0.16)×10-2mmHg-1,P﹤0.05]。直线相关分析显示,脉压与颈动脉僵硬度成正相关(r=0.552,P﹤0.01),与颈动脉扩张性成负相关(r=-0.552,P﹤0.01)。结论原发性高血压患者脉压与颈动脉功能改变有关,脉压增高可使颈动脉僵硬度增高、颈动脉扩张性降低。
Objective To investigate the correlation between pulse pressure and carotid artery function in patients with essential hypertension. Methods A total of 98 patients with essential hypertension were selected. According to the results of ambulatory blood pressure monitoring, patients were divided into two groups: pulse pressure> 60mmHg and ≤60mmHg. The carotid artery stiffness and carotid artery dilatation were measured by high frequency ultrasound. Results The carotid artery stiffness was (1437.33 ± 497.41) mmHg in pulse pressure> 60mmHg group, significantly higher than that in pulse pressure ≤60mmHg group (843.46 ± 214.72mmHg) (P <0.05), and the carotid artery distensibility was significantly lower (0.21 ± 0.11 ) × 10-2 mmHg-1vs (0.39 ± 0.16) × 10-2 mmHg-1, P <0.05]. Linear correlation analysis showed that pulse pressure was positively correlated with carotid artery stiffness (r = 0.552, P <0.01), and negatively correlated with carotid dilatation (r = -0.552, P <0.01). Conclusion The pulse pressure of patients with essential hypertension is related to the change of carotid artery function. Increased pulse pressure can increase the stiffness of carotid artery and decrease the carotid artery dilatation.
