目的:探讨妊娠早中期血常规白细胞(WBC)总数、血小板(PLT)计数、红细胞(RBC)计数及血红蛋白(HGB)含量变化与妊娠糖尿病(GDM)发生的相关性。方法:分别在孕妇首次孕检即孕12~14周及孕24~28周行糖筛查时,予血常规检查,测定WBC、PLT、RBC及HGB。将葡萄糖糖耐量检查确诊为GDM的78例孕妇作为病例组(GDM组),糖耐量正常的683名孕妇作为对照组(Con组),分别比较两组早、中期WBC、PLT、RBC等计数及HGB水平与GDM发生的相关性进行统计学分析。结果:GDM组早期外周血白细胞计数、红细胞计数、血红蛋白明显高于对照组(P<0.05),血小板计数无明显差异。GDM组中期外周血红细胞计数、血红蛋白含量明显高于对照组(P<0.05),中期两组外周血白细胞、血小板计数均升高,无统计学差异。GDM组早中期血小板计数明显下降,患者高危并发症明显增加。结论:孕早期外周血白细胞计数、红细胞计数、血红蛋白含量明显升高与GDM患病正相关(P<0.05);孕中期外周血红细胞计数和或血红蛋白含量升高与GDM患病正相关(P<0.05);GDM患者血小板计数下降是疾病的独立危险因素。
Objective: To investigate the correlation between the change of gestational diabetes mellitus (GDM) and the number of WBC, PLT, RBC and HGB in early and middle gestation. Methods: WBC, PLT, RBC and HGB were measured during the first pregnancy test of pregnant women, ie 12-14 weeks pregnant and 24-28 weeks pregnant respectively. A total of 78 pregnant women with GDM diagnosed with glucose tolerance test (GDM group) and 683 pregnant women with normal glucose tolerance as control group (Con group) were enrolled in this study. The WBC, PLT, RBC, HGB levels and the occurrence of GDM correlation analysis. Results: The number of peripheral blood leucocyte, red blood cell count and hemoglobin in early GDM group were significantly higher than those in control group (P <0.05). No significant difference was found in platelet count. The number of peripheral blood erythrocytes and hemoglobin in the GDM group were significantly higher than those in the control group (P <0.05). There was no significant difference in peripheral blood leukocytes and platelet count between the two groups. GDM group early and mid-term platelet count significantly decreased, patients with high-risk complication increased significantly. Conclusions: Peripheral blood leukocyte count, erythrocyte count and hemoglobin content in early pregnancy are positively correlated with GDM (P <0.05). The increase of peripheral blood red blood cell count and / or hemoglobin in the second trimester is positively correlated with the prevalence of GDM (P < 0.05). Decreased platelet count in GDM patients was an independent risk factor for the disease.
