目的分析血清脂联素、胰岛素抵抗与代谢综合征(MS)的相关性,为糖尿病管理提供依据。方法 136例MS患者作为MS组,136例体检完全不具备代谢综合征任一组分的正常者作为对照组,进行相关检查,统计数据进行比较分析。结果 MS组年龄、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖、空腹胰岛素、餐后2 h血糖、甘油三酯、稳态模型胰岛素抵抗指数(HOMA-IR)水平高于对照组,高密度脂蛋白、脂联素低于对照组,差异均有统计学意义(P<0.05);对上述指标进行Logistic多元回归分析,年龄、BMI、HOMA-IR、脂联素成为独立影响因素,OR值分别为1.024、1.475、2.673、3.190;脂联素、HOMA-IR负相关(r=-0.571,P=0.01<0.05)。结论血清脂联素的下降可能是胰岛素抵抗上升的影响因素,可作为监测指标,指导疾病管理。
Objective To analyze the correlation between serum adiponectin, insulin resistance and metabolic syndrome (MS), and provide the basis for diabetes management. Methods One hundred and sixty-six patients with MS were selected as the MS group, and 136 healthy subjects without any component of metabolic syndrome were selected as the control group. The related tests and statistical data were compared and analyzed. Results There were significant differences in the age, BMI, DBP, fasting blood glucose, fasting insulin, 2 h postprandial glucose, triglycerides, homeostasis model insulin resistance index (HOMA-IR ) Were higher than those in the control group, high density lipoprotein and adiponectin in the control group (P <0.05). Logistic multiple regression analysis showed that age, BMI, HOMA-IR, Interleukin was an independent influencing factor, with OR values of 1.024,1.475,2.673,3.190 respectively; negative correlation was found between adiponectin and HOMA-IR (r = -0.571, P = 0.01 <0.05). Conclusions The decrease of serum adiponectin may be the influencing factor of the rise of insulin resistance, which can be used as the monitoring index to guide the disease management.
