目的探讨人附睾蛋白(HE4)、膜联蛋白A3(Annexin A3)、间皮素(Mesothelin)在上皮性卵巢癌(EOC)血清与组织中的表达及其对卵巢癌铂类耐药的影响。方法选取2011年9月-2014年3月该院妇科收治的78例EOC为研究对象。按铂类药物疗效分为铂类敏感组(62例),及铂类耐药组(16例)。ELISA法检测78例EOC患者血清HE4、Annexin A3、Mesothelin及CA125水平,免疫组化法检测组织中HE4、Annexin A3和Mesothelin蛋白的表达,分析HE4、Annexin A3和Mesothelin与卵巢癌铂类耐药的相关性。结果 78例患者中两组间血清HE4、Annexin A3、Mesothelin、CA125浓度差异均有统计学意义(P<0.05);血清HE4、Annexin A3和CA125阳性表达率差异有统计学意义(P<0.05),血清中4种蛋白的阳性表达两两比较均呈正相关(P<0.05),HE4、Annexin A3、Mesothelin在EOC组织中的阳性表达率差异均有统计学意义(P<0.05),78例组织中HE4、Annexin A3、Mesothelin阳性表达两两比较,差异均有统计学意义(P<0.05);HE4、Annexin A3、Mesothelin在血清和组织中的阳性表达均呈正相关(P<0.05)。结论 HE4、Annexin A3和Mesothelin参与了卵巢癌发生发展,并与铂类耐药密切相关;联合检测其在血清和组织中的表达有助了解卵巢癌铂类耐药的可能机制,预测化疗敏感性,并可为铂类耐药性卵巢癌的治疗提供靶点。
Objective To investigate the expression of human epididymis (HE4), annexin A3 and mesothelin in the serum and tissues of epithelial ovarian cancer (EOC) and its effect on platinum resistance in ovarian cancer. Methods From September 2011 to March 2014, 78 cases of EOC received gynecology in our hospital were selected as research objects. According to the efficacy of platinum drugs were divided into platinum-sensitive group (62 cases), and platinum-resistant group (16 cases). Serum levels of HE4, Annexin A3, Mesothelin and CA125 in 78 patients with EOC were detected by ELISA. The expression of HE4, Annexin A3 and Mesothelin in tissues were detected by immunohistochemical method. The expression of HE4, Annexin A3 and Mesothelin in ovarian cancer was analyzed. Correlation. Results There were significant differences in the concentrations of HE4, Annexin A3, Mesothelin and CA125 between the two groups (P <0.05). The positive rates of HE4, Annexin A3 and CA125 in serum were significantly different (P <0.05) (P <0.05). The positive expression rates of HE4, Annexin A3 and Mesothelin in EOC tissues were significantly different (P <0.05), 78 cases of tissues The positive expression of HE4, Annexin A3 and Mesothelin in each group was statistically significant (P <0.05). The positive expression of HE4, Annexin A3 and Mesothelin in serum and tissues were positively correlated (P <0.05). Conclusions HE4, Annexin A3 and Mesothelin are involved in the development of ovarian cancer and are closely related to platinum resistance. Combined detection of their expression in serum and tissues may help to understand the possible mechanism of platinum resistance in ovarian cancer and predict the chemosensitivity , And can provide a target for the treatment of platinum-resistant ovarian cancer.
