本文对66例脑血管畸形的临床影像、TCD进行了对比分析;并以166名健康人各年龄组TCD各指标值进行对照;另将其中30例经X刀立体定向放射治疗的结果,进行了为期2个月到3年的随诊观察,并结合治疗前、后临床、TCD、DSA、MRA对比分析。所用TCD仪为EMETC2020型,定向立体X刀放射治疗仪为菲利浦SRS200型。选择病例均以多次人工栓塞术后无效者,畸形血管因直径大小以4cm以下者为主体,最大者可达8cm×4cm×3cm。66例中男40例、女26例,年龄<20岁16例,21~40岁38例、41~60岁以上12例。病种分类:动静脉畸形(AVM)者59例,海绵状血管瘤2例,Moyamoya氏病5例。部位分布:幕上55例,幕下6例、脑底5例。各脑叶分布:额18例、颞15例、顶19例、枕10例、基底节、丘脑6例、小脑5例、胼胝体1例、中脑、脑室1例、脑底环5例。按血动脉分布:基底动脉系统10例、颈动脉系统56例,其中以MCA、ACA为主要病变动脉。按AVM血管团直径大小分类:<2cm,23例、2.1~4cm,34例、>4.1cm者,4例、脑底动脉网5例。影像诊断:CT脑扫描23例、MR29例、MRA7例、DSA?
In this paper, 66 cases of cerebrovascular malformations of clinical imaging, TCD were analyzed comparatively; 166 cases of healthy people in each age group TCD control values; the other 30 cases by X-knife stereotactic radiotherapy results were A 2-month to 3-year follow-up observation was performed and combined with clinical, TCD, DSA and MRA before and after treatment. Disease classification: arteriovenous malformations (AVM) in 59 cases, cavernous hemangioma in 2 cases, 5 cases of Moyamoya’s disease. Location distribution: 55 cases on the screen, 6 cases under the screen, brain in 5 cases. There were 18 cases in each brain, 15 cases in temporal, 19 cases in top, 10 cases in occipital, 6 cases in basal ganglia, 6 cases in thalamus, 5 cases in cerebellum, 1 case in corpus callosum, 1 case in midbrain and brain and 5 cases in brain base. By blood artery distribution: basilar artery system in 10 cases, carotid system in 56 cases, of which MCA, ACA as the main lesion artery. According to the size of the AVM vascular diameter: <2cm, 23 cases, 2.1 ~ 4cm, 34 cases,> 4.1cm, 4 cases, cerebral artery network in 5 cases.
