本文报道了1例老年男性患者，咳嗽、气短半年就诊。血MPO-ANCA阳性。胸部CT显示双下肺以及胸膜下为主的网格影、蜂窝肺；外科肺活检病理符合UIP，伴较多淋巴细胞浸润及淋巴滤泡形成，考虑继发性。3年后出现肾功能损害，肾脏穿刺活检提示坏死性小血管炎肾损伤，诊断为显微镜下多血管炎。肺部病变考虑显微镜下多血管炎继发间质性肺炎。
