目的 探讨血栓性血小板减少性紫癜 (TTP)的临床诊断及治疗对策。方法 以TTP五联征或三联征为诊断主要依据 ,采用以血浆置换为主的综合治疗。根据Lichtin疗效标准判断预后。结果 17例TTP早期误诊 8例 ,死亡 8例。 11例采用以血浆置换为主的综合治疗 ,置换次数为 3～ 15次 ,9例治愈无复发。结论 根据临床征象、微血管病性溶血、血小板减少、血清乳酸脱氢酶等能获得诊断 ;早期采用综合治疗 ,尤其注重血浆置换及血浆输注将明显改善其预后。
Objective To investigate the clinical diagnosis and treatment of thrombotic thrombocytopenic purpura (TTP). Methods TTP five levy or triple sign for the diagnosis of the main basis for the use of plasma-based comprehensive treatment. According to Lichtin efficacy criteria to determine the prognosis. Results 17 cases of early TTP misdiagnosis in 8 cases, 8 died. Eleven patients were treated with plasma exchange, the number of replacement was 3 to 15 times, and 9 patients were cured without recurrence. Conclusion According to clinical signs, microangiopathy hemolysis, thrombocytopenia, serum lactate dehydrogenase can be diagnosed; early comprehensive treatment, with particular emphasis on plasma exchange and plasma transfusion will significantly improve its prognosis.
