目的评价计算机辅助设计和自制牙支持式导板应用于下颌后牙区种植的临床疗效。方法选择2012年4—9月山西医科大学口腔医院口腔外科收治的下颌后牙区单颗缺失患者10例,制取缺牙区牙列模型,选取植入点,制作初导板。采集CT数据,利用Simplant软件对数据分析,使用研磨仪在模型上确定最后植入方向并制作最终导板。口内戴入导板,植入种植体,拍摄CT,测量术前术后角度及深度偏差。结果种植体在颊舌向与模拟植入体方向存在偏差,但偏差角度在10°以内,可以通过角度基台调整以适应咬合关系。种植深度及近远中向角度术前、术后偏差较小。结论该系统可指导临床医生在下颌骨高度不足条件下植入种植体,对于提高种植手术质量与精度具有重要意义。
Objective To evaluate the clinical efficacy of computer-aided design and self-made tooth-supported guide plate in mandibular posterior teeth. Methods Ten patients with single deletion in the posterior mandibular area admitted to Department of Stomatology, Stomatology Hospital of Shanxi Medical University from April to September, 2012 were selected to establish the dentition model. CT data was collected, analyzed for data using Simplant software, and the final orientation of the implant was determined on the model using a grinder and a final guide was made. Oral wear guides, implants, CT, measurement of preoperative and postoperative angle and depth deviation. Results The implants were deviated in buccolingual direction and simulated implants, but the deviation angle was within 10 °. The angle abutment could be adjusted to fit the occlusal relationship. The depth of implantation and mesiodistal angle of preoperative and postoperative deviation is small. Conclusion The system can guide clinicians to implants implants in the condition of insufficient mandibular height, which is of great significance for improving the quality and precision of implants.
