目的探讨基质金属蛋白酶9 (MMP 9)在分娩发动、宫颈成熟和扩张中的作用,以及地西泮促宫颈成熟和扩张的机理。方法采用酶联免疫吸附试验(ELISA)测定有宫缩的56例剖宫产(有宫缩组)产妇不同产程及无宫缩的16例剖宫产(无宫缩组)产妇子宫下段组织(含蜕膜及子宫肌组织)中MMP 9的水平,以及在子宫下段组织成纤维细胞中加入不同浓度的地西泮(0、0 1、10、100μmol/L)进行体外培养,半定量RT PCR法测定成纤维细胞内MMP 9mRNA的表达,ELISA法测定成纤维细胞上清液中MMP 9的水平。结果有宫缩组产妇子宫下段组织中MMP 9的水平为(3 1±2 6)ng/mg组织,明显高于无宫缩组的(0 8±0 4)ng/mg组织,两组比较,差异有统计学意义(P<0 05)。有宫缩组第一产程44例产妇随宫口开大,MMP 9的水平呈增高趋势,平均为( 2 2±2 0 )ng/mg组织。第二产程12例产妇MMP 9的水平为( 6 1±2 5 )ng/mg组织,明显高于第一产程(P<0 05)。体外培养的成纤维细胞内MMP 9mRNA的表达水平,在地西泮0 1、10μmol/L浓度及空白对照浓度(浓度为0 ),均呈时间依赖性变化,即随着成纤维细胞培养时间( 3、6、12、24h)的延长,MMP 9mRNA水平表达逐渐升高。成纤维细胞上清液中MMP 9水平在地西泮0 1μmol/L浓度与空白对照,也呈时间依赖性变化,即随着细胞培养的时间延长而水?
