目的探究小剂量与常规剂量甘露醇颅压降幅的差异及肾功受损的发生率。方法采用蛛网膜下腔置管的方法,实时动态监测小剂量和常规剂量甘露醇分别在颅压轻度增高组和显著增高组不同时段的颅压降幅。结果在颅压轻度升高组,小剂量与常规剂量的颅压降幅差异无显著性(P>0.05);在颅压显著升高组,二者的颅压降幅差异具有显著性(P<0.05);肾功损害的发生率,二者差异无显著性(P<0.05)。结论甘露醇剂量的选择应视颅压增高的程度而定,对颅压显著增高的病人宜选用常规剂量。
Objective To investigate the difference between the decrease of mannitol intracranial pressure and the incidence of renal damage in small dose and conventional dose. Methods The method of subarachnoid catheterization was used to dynamically monitor the decrease of intracranial pressure in different periods of mild pressure increase and low pressure increase of low dose and normal dose of mannitol in real time. Results There was no significant difference in the decrease of intracranial pressure between mild and moderate doses (P> 0.05). There was a significant difference between the two groups in the significant increase of intracranial pressure (P < 0.05). There was no significant difference between the two groups in the incidence of renal damage (P <0.05). Conclusion Mannitol dose should be selected depending on the extent of increased intracranial pressure, the intracranial pressure was significantly higher in patients should choose the conventional dose.
